South Mountain Post Acute, Phoenix AZ

I just completed my rehab for my leg operation at this facility. Very clean and great staff that work here. Nursing and therapy were top notch, highly recommend.

It was my first time coming the nurses made me feel comfortable Tay an Damita went out of her way to make sure I was good an help me I recommend ppl coming to this rehabilitation center

Don’t send your loved ones to this place. Per my Aunt’s recent stay and experience;
Well my family and friends I am home now and so glad to be home at that. If I ever have to go to a nursing skills facility again, believe me, it would not be South Mountain Post Acute. I am never going to go to that place again. Some of the nurses are so rude and mean. I called for help cuz I needed help to situate myself or turn on the bed and one nurse told me, I'm not gonna lift you, it would take 10 people to lift you! " They take such a long time to come help you and then they are so eager to come and turn your call button light off without asking you what you need. On the night shift they have one nurse taking care of 26 patients so you might as well forget about them coming to help you anytime soon. You can be in excruciating pain asking for your pain meds and all they tell you is, "the nurse is coming around with the meds, you just have to be patient" and you wait and wait and wait! The meals always come to you cold and who likes to eat cold oatmeal, so they bring you your oatmeal but no milk and no sugar to put in it. You always have to ask for milk and sugar. At night you have to ask for a snack, then you ask for milk and they never have any milk at night. Believe me there can be a number of patients crying bc they need help and they take forever to come, they just come and turn the call button light off and leave, then they say, I'll come back in a minute and a minute can be an hour or two long. One nurse even forgot to come back. Me and my roommate did our share of crying. And how about when you need a bed pan, same thing, you desperately need to use the restroom but you can't get up, so you ask for a bed pain and you are there holding it forever till they come help you. So they put you on the bed pan and then you have to wait another hour for them to get you off the bed pan. So South Mountain Post Acute is definitely off my list of places to go too when in need. Also the Aide that packed my stuff to come home didn't empty the water reservoir from my bipap machine, the machine got all wet and now it doesn't work. Good thing to have an old machine here at home that I was able to use. She also broke off the climate air heating oxygen port that connects to the oxygen concentrator to give me oxygen while I sleep. So I slept with without oxygen last night and now I have to wait till Monday to order one and that costs $67.00. Hopefully Preferred Home Care can send me one free. Sleeping without oxygen can be detrimental for me cuz I have sleep apnea and can stop breathing. Believe me I am never going to that place again.

South Mountain has been a pleasure to work with! Responsive staff. Pleasant atmosphere. I’ve been impressed!

Love working here! Clean facility, friendly nursing and therapy staff, new market for everyone to enjoy. I have been here for a year and a half and have seen so much remodeling done to make it a better place for our community. If your family member needs a skilled nursing or long term care placement please come see us as we are happy to give you a tour and help you make a decision.

South Mountain has a great team of staff that focus on getting the job done and done right! The facility is clean and smells great. I am always greeted by a staff member with a smile.

South mountain is a place with wonderful staff that I can call family they are very caring and the facility is nice and clean. All employees go above and beyond to care for the residents there. I am glad to be part of the team

New to facility but by far the best facility I have ever worked at. Very clean, staff are very approachable and will really make you feel welcome. Learning new things everyday with the leadership team. Looking forward for a long term stay with SMPA!

South mountain is a great facility. It's very nice, clean and welcoming. The staff there are angels. They are truly unsung heroes. Marianne is the director there and she is so caring, her staff goes above and beyond. They all deserve a raise in my eyes. The administrator Courtney really cares about the patients. He listens. My mom hates dark chocolate and here comes Courtney with the white chocolate. Those small things really help a person heal!

Highly recommend South Mountain, the staff treat their patients with respect and care for them as if they were their own family members! A+

Loved being a patient, was treated like gold after bypass. I go for outpatient therapy also, love it.

Very clean and staff is very attentive to details of their patients. Very happy with the care that I have received from them. Tyler is very personable and makes sure that your care is being well taken care of. Highly recommend this facility.
