I believe reviews are essential for providing valuable insights into a facility, helping others make informed decisions about the care and services offered. My father was admitted to Tempe Post Acute on October 11,2023.
I want to express my appreciation for the hardworking nurses and CNAs at this facility, as they truly are the backbone of its operations. Tempe Post Acute is a facility of decent size, extending its services to patients from Yuma and other cities. However, I must highlight some concerns about the overall cleanliness of the facility and persistent odors in the hallways. Proper ventilation may be the cause if certain cleaning agents are causing these issues.
During my father's stay, there was a large dog freely roaming, which could be unsettling, and this was not even for a blind patient. Additionally, I observed a lack of housekeeping staff, with nurses and CNAs handling trash removal.
Each room in the facility accommodates two patients in a room, sharing a single bathroom for two rooms. This setup can lead to challenges with maintaining cleanliness, especially when four different patients are utilizing the same bathroom.
One notable issue was the absence of doctor visits during my father's stay. Despite requesting an ultrasound for his swollen left leg, there was a delay in follow-up. When the ultrasound results returned negative, we expressed our ongoing concerns, only to be informed that a doctor wouldn't be available until Monday. Faced with this situation, my siblings and I opted to take our father to the hospital for immediate attention, as waiting until Monday seemed impractical.
Regrettably, when we informed Tempe Post Acute about our decision, they indicated they wouldn't readmit my father if we left willingly. This raised concerns about the facility's management and its commitment to customer care.
We took our father to the hospital at 1: 30 AM on Saturday, October 21st, 2023. According to the hospital ultrasound, he had several blood clots in his left leg, including a few in his lungs. The hospital staff agreed with our decision to bring him here, stating that waiting until Monday would have resulted in a worse condition for my father.
I advise prospective residents and their families to thoroughly assess the cleanliness and overall conditions of Tempe Post Acute or any nursing facility before making a decision. It's crucial to ensure that the chosen facility can provide the level of care and cleanliness your loved ones deserve.

I had high hopes for this place. My 85 year old father-in-law ended up in the hospital with a broken rib. He was sent here for PT before returning to his permanent care facility. In their defense, he does have Stage 5 Parkinson's but he does have some mobility. At TPA, other than the one hour of PT (which was NOT daily), he was confined to his bed. They used a lift to get him in and out of bed instead of working with him to help himself get out of bed. They did the same for the bathroom; just put a Depends on him and wouldn't let him shuffle his way to the bathroom. Very disappointed. I know they have a lot to do, but they weren't helping him. They were hindering him. And, he wasn't give his CPAP for multiple nights so he had very little sleep and ended up in the ER again. At that point, we decided to move him to his normal facility and set up PT and OT there. Just one night of having his CPAP back he is back to normal. While he can't move much, at least he's at his normal place now that encourages movement with people that want to help him.

Dealing with medical is extremely stressful. So often we feel as though we have no control of the situation. This facility is staffed by absolute professionals who guide you through the process and are there to offer support and direction.

Super friendly/great staff I came in with a walker and now thanks to physical therapy I can walk without one!

Horrible place to put your loved ones. A few good nurses. Leaves your family in soiled briefs for hours. Gives your family soured milk and the therapists are rude.

Zero stars. Patients are constantly ignored by nurses. Definitely do not recommend this place for your loved ones. Horrible care. Don't bother calling the switch board. They don't transfer correctly and they also hang up on you. Very very very poor service

Hello I would like to come on here to tell all of you who are even considering this place to look else where. We were debating on sending my grandmother here as my friends mom was going here at the time but after hearing what she dealt with, I don't think this place should even be open. I got note that one of these employees are asking people to leave a good review for money. This place is terrible and money driven. To pay someone to leave a good review! Are you kidding me. This is actual people's lives and reality. Shame on you. If I could give this place a 0 I would.

The nurses have no emotional regulation and when they get mad they refuse to feed their patients. They also send patients to the hospital without telling the patient's emergency contact and then withhold information from that person to the point of them having to call all the local hospitals to locate them.

This place is alright. Staff is a lil moody. But they do have 24/7 visitation. So if the lady at the front desk says visitation is from 8-8.then she's lying

! WARNING! DO NOT SEND YOU FAMILY OR LOVED ONES TO THIS PLACE FOR SKILLED NURSING! This facility is old and run down. The CNA staff are the only ones that try and just barely. The rooms are dirty and when they clean its run a dirty mop on the floor. The night staff is the most uncaring, unprofessional, and unethical medical people I have ever been involved with. I would visit my father and push the call button for help it would take 45 minutes to 1 hour for someone to come. They would show up annoyed you pushed the button. The food is awful I wouldn't even give it to my German Shepard. I went to pick my father up and it was the epitome of inefficiency. I had to get on them to get him ready since of course he wasn't ready at the scheduled time. The case manager tried to help but she is way overworked and most likely underpaid. Go to Yelp for the real reviews instead of these fake ones here.

Competent and caring nurses who contribute to the overall well being of the residents. Nurses like Jordan provide exceptional wound care/treatments and utilize their skills to improve quality of care and life of the residents. Highly organized, team player, and natural born leader are some more qualities I have observed. The team is in good hands with such qualified and compassionate nurses.

Stayed there back in July. Management staff is very helpful. Also the CNAs are great with the exception of one or two but that's the case anywhere. PT and OT are good but I didn't have enough time there based on my insurance. However, the real winner here is the manager of the food! Not only were the meals larger and better than some other facilities, she went above and beyond to create an entire menu of vegetarian items just for me that I assume are now available to everybody. And, they were really delicious. I felt bad when I abruptly discharged and didn't have a chance to thoroughly thank her and the CNA's who made themselves available to me. Not as bad as some people are claiming. They probably just had the wrong expectations if transferring from a larger facility/hospital. Thanks everyone

Nurses/Staff were always extremely helpful & had their best interest towards the residents. Tempe Post Acute did a very great job taking care of my father & I felt at ease knowing the level of care he was receiving while I was busy working. I would highly recommend this facility to anyone.

I call, it takes a long time before anyone answers and they use the excuse that they are short staff. When I arrive they are sitting talking about an incident that happened to my sister. My sister was to have surgery and they fed her a full meal. When it was time to pick her up no one mentioned the meal she ate. Also she was dirty, she had feces and no one changed her untill five minutes before she was going to get picked.up Also there was no mentioned or documentation given to me or surgeon that had eaten. She did not have her procedure done because they had given her a full meal When asked the nurse said she was not her nurse. She was prepped for the surgery and questions were asked The surgeons staff found a out they had given her pills and a full meal so the procedure was cancelled.it was a very bad experience.
