My mom is receiving care at this facility. Her nurse Pam has been so patient and kind. Rachel in activities set up a visit in the patio and allowed some family members to join us. It was a good day!

This facility is the best facility I have seen in years! Took extremely good care of my mother and helped me bring her home. The staff was extremely helpful to me and the cna’s went above and beyond for my mother. Highly recommend this place!

I have nothing but amazing things to say about this place! The CNA’s are so sweet and go above and beyond for my grandma. Station 3 I recommend they took care of her and no complaints!

I usually don’t write reviews but I felt like I had to, my grandfather had been cared for here and really enjoyed the care he received. The staff is very helpful and kind and treat their patients very well. My grandfather has been cared for at another acute but this one was by far the best. I would recommend this place to everyone honestly!

My grandma was taken really good care. Their priorities and attentions are targeted to every patient. Very clean clinic and great staff.

This facility helped my grandma get back home after COVID. When she first got diagnosed with COVID we were so scared we couldn’t bring her house, and no other facility would accept her due to her insurance until avocado came to the rescue. So thankful for the staff!

Very bad experience, the facilities are not very good, it is very difficult to communicate with your family member, when you want to get her out of there they put many obstacles to the degree of denying the patient medications.

Advice do not leave your family member with them.

It is a very poor quality clinic, poor service from nurses, social workers and managers.
Terrible service to be able to communicate with your family member since they only have one phone for all patients.
My grandmother was hospitalized there and they did not facilitate her personal hygiene, her appearance looked neglected.
When we decided to get her out of there they put a lot of obstacles on us and when you asked why you couldn't take her they simply threatened me that if I took her they would not give me the medicine that was being administered.
Honestly, I would never ever put her there again, it is a place of very poor quality.

This clinic has been very helpful and their workers are very kind. My grandmother has said nothing but good things about the staff and how they treat her with a gracious amount of kindness. I wouldn’t recommend any other facility to take care of loved ones other then this one.

Do not admit anyone from your family to this clinic, it is very unprofessional, apart from the fact that it is in a bad state, the service they have is terrible, it is very complicated when you want to communicate with your family member, the nurses have a very badly attitude and they threat you, when you want to get your relatives out it is very complicated, they put many obstacles on you.
My grandmother is in there and they don't let us take her home.

No admitas a nadie de tu familia en esta clínica, es muy poco profesional, aparte de que está en mal estado, el servicio que tienen es pésimo, es muy complicado cuando quieres comunicarte con tu familiar, las enfermeras tienen muy mala actitud y te amenazan cn no darte los medicamentos, cuando quieres sacar a tus familiares es muy complicado, te ponen muchos obstáculos.
Mi abuela está ahí y no nos dejan llevarla a casa.

Do not send anyone here. My father in law was here for a week. He was not able to feed himself, so he starved. He was not able to shave himself, so he did not get shaved. They lost one of his hearing aids. They didn't dress him in his clothes that he brought. We are filing elder abuse with the county Welffilingfiliqbuseabuse with the cointy

I am a nurse who works for a hospice company. I visit a lot of different nursing homes to see patients and I have to say this one is really good. The staff do a good job caring for the patients and the patients are always presentable and happy. I’m impressed by the staff here.

My aunt just discharged from the the subacute here and she had nothing but positive things to stay about the staff. She wanted me to say a special shout out to the charge nurse T and respiratory therapist Jason. She said everyone was great but these two really helped her out so much. She’s happy to be home but will miss the kind people there.

The staff at this facility do an excellent job rehabilitating and providing quality care to the patients there. From the administrator to the housekeeping staff they go above and beyond to make sure the patients are happy and comfortable.

One of the worst facilities I’ve ever known. It appears to be a training facility. Under staffed. Med were awful. They kept trying to give me medicine I was allergic too and when I asked for tums you would think I asked for something very serious. Lastly there are very few that were especially good. The end

Facility is very clean staff was very friendly. It’s been a couple weeks and my mom has progressed so much. The Director of therapy Ann Marie is very nice. Ariana was very helpful throughout the whole process and even gave me her personal cell phone number

If I could leave 0 stars I would! This place is the absolute worst! My father has been time and time again neglected by the nurses and staff! One nurse when administering a shot with medication, my father had to point out she hadn’t sterilized the area where the shot was going to be given. To which the nurse replied, “ARE YOU REALLY GOING TO MAKE ME GO GET A ALCOHOL SWAB? ” The fact that my father needs to remind the staff to feed him! The Medical Board should be looking into this facility! Luckily my father is able to speak up, I can’t imagine the patients who aren’t able to. All around terrible facility, ALL BAD!

I had a family member here that I cannot get a hold of. I spoke to Kaylen I believe was her name. She was very sweet, caring, and helpful. You can clearly tell they are short staffed just by the tone in the nurses voices. They never have anyone in the stations answering phones. As a fellow Nurse there has to be a clerk at the stations at all times receiving calls. It may not sound like a big deal, but miscommunications will cause big problems with family members and staff.

If I could give this place a negative star I would my mom has been sent there and I should of read the reviews before her going there. There is 5 sibling and only 2 have been able to get a hold of her and that is a hand full of times that they spoke to her. They don't answer the phone and when you get transferred the phone rings 3 times and hangs up. I got 1 sweet young lady Kayla that seemed concerned but to my luck when she transferred the call to nurse station the call got hung up after the 3rd ring. There is no phones in the rooms so you have to rely on the nurses to get them the phone. My mom had a cell phone and now she can't find it. Horrible if you ask me! I hope she recovers soon so she can get the hell out of there.

Worst place ever I would not recommend it. They are very poor in communications and take very long time to answer phone calls my father is their as a patient and he's been hurt various times do to their bad service.
