Thank you to Cam, the Director, and staff for efficiently handling Mom's transition to Assisted Living. The care and follow through on all the details made the tedious process smoother for us so we could focus on Mom. Thank you!

La Casa Via's service is terrible. My therapist (John Muir) ordered an AFO brace from them in 2019. One year ago (2021) I found the belt of my brace was damaged. I went to their office and asked for the fixing. They refused to fix it and told me that they couldn't find my name in their computer. Recently I told my therapist that what happened. My therapist called them and they asked me to go to their office again. But when I went to their office they said their computer has a problem and couldn't find an appointment with them, so they asked me to go to their office again on next Friday.
Having a stroke is painful and still facing to this kind of service is so sad. This is grateful if this company can improve their service.

The food is very good, much improved since last December. The staff continues to be caring, response good 70% of time, lower if emergency help needed on floor. Update after a few more days, that’s the plan

On February 12,2022 we were in a crisis situation with our family member and desperately searching for a safe location. They had been pre-maturely discharged from a Kaiser facility with zero support. We were incredibly blessed to have Cam be at La Casa Via when we called. After grasping out situation (and we are out of state), she gave us her cell number, answered every question we had, maintained contact throughout the weekend and was able to admit our family member by 2pm on Monday.
Throughout the time at La Casa Via, Cam and her team have been amazing resources. They are quick to communicate and give us details on progress. We are a team and we both have the well-being of our family member as our common goal.
Even though we are out of state, we have been able to visit La Casa Via and observe it's caregivers and therapy team. They are a small facility, but they are kind, friendly and professional.
We are so thankful that we connected with them. Thank you Cam, Lupita and the rest of the La Casa Via team.

I visited this facility. It is dirty and old. The care staff wasn't very friendly and the promises they make, never able to deliver. There are plenty better options out there. Since it Medicare money, look around for the best place for your loved one. Don't let hospital social worker push you to be at this filthy place.

My son was taken here after a car accident. It became frustrating when nurses were telling me one thing, my son another. The doctor never returned a call. I was told my son needed memory care since they told me he couldn't remember anything, and he couldn't get out of bed or move without assistance.
My son was adamant they were wrong. After going around with different staff members, and getting different answers I pulled him out.
His memory is as sharp as a tack, and he was able to walk on his own with a walker. Why the lies?
I plan on sending a letter on to the director, or owner, and let them know everything that way done, and not done. I was very disappointment.

I recently spent a month at this nursing home after fracturing my kneecap. All the staff treated me like a guest in their facility! The CNAs and nursing staff were very attentive and the physical therapists helped me each day to move forward with my rehab. The food was also very good. The managers Cam and Lupita were always available to help with special requests. My thanks to all of you for making my first stay at a nursing home enjoyable!

I was a patient at Kindred in June 2017. I was a serious fall risk, having a brain condition that causes strokes which I was recovering from when I was transferred there from acute inpatient care in ICU in John Muir Hospital. After a few days at Kindred where I had been requesting bedrails because of my being a serious fall risk, and Kindred had none.
They ordered them but had to wait several days to get them!
Meanwhile one morning I turned in my bed and slipped off the head of it onto the floor.
Ambulanced back to JM which I do not remember. Report says I had two critical brain hemorrhages. My brain MRI's show that I am Still suffering from this. It seems to be aa permanent thing.

I just saw this review of Kindred where I was a patient in June of 2017. The "Kindred" skilled nursing facility I was in was in the Rossmoor area, not the JM Hospital area. I think there must have been another "Kindred" SNF in the hospital area. In any case the Kindred I was in was later taken over by new management, partially perhaps because of mine and others' reports of atrocities such as dangerous conditions such as lack of bedrails even when requested as I and my daughter had done, since I had just been transferred from JM hospital where I was definitely a big fall risk. (and I my regular brain MRI's DO still show that I have lasting brain damage from the fall there and the huge brain hemorrhages that resulted.)

The staff at La Casa Via is great. They took in my Dad for the last 5 months of his life and treated him like a king during every phase to the end of life. I knew Cam when she was at Tampico and took care of my mother and father many times in rehab. Her and her staff did a wonderful job and I really appreciate their dedication to the clients.

I truly love this care home, all the employees so caring, always there to help and accommodate you in anyway possible. I will always refer residents to come here as I feel it’s the best care for short term and long term care can’t say enough how wonderful it truly is.

I can’t say enough about Cam and the staff at La Casa Via. Our family has had 2 emergency situations in the last year, one with our father and then more recently with our mother. She guided us through a very difficult process with compassion and very personalized attention. She is an expert in her field and was able to help us find the care we needed.
Our dad stayed for 2 weeks after an emergency neck surgery. He still talks fondly of Cam and the staff a year later. He was able to get back on his feet quickly.
You can tell how invested she is in helping people by how she goes above and beyond. She is currently helping us maneuver through a challenging care situation with our mother after a fall. As soon as we contacted her (via text on a weekend, btw), she became an advocate for our mom. If you are looking for someone that truly cares about the families she helps, reach out to Cam and La Casa Via.

La Casa Via is a wonderful nursing facility located next to John Muir Hospital. Cam and the staff are absolutely amazing. Everyone that works here is always smiling and the attention to detail is great. Facility is always clean and smells great. My Dad is well taken care of and all his medical and social needs are met above expectations. Physical therapy and all other medical and therapeutic services are accessible and well staffed. Surely management takes great care in ensuring continuity throughout. Thank you Cam and the team at La Casa Via.

I just wanted to give a shout out to Cam and the staff at La Casa Via. I am a 50 year old women who in the past 6 months has had to deal with an array or tragedy! The least was a hip replacement, so when I made the discussion to have the surgery I knew I needed a rehabilitation hospital so I would heal properly. I never in a million years would have believed the care and service I received from the staff there! They were kind and reassuring to all the older patients there and myself! I worked at Nordstrom years ago and l have seen anything close to there service until I checked in there. The employees went out of there way to provide home like environment. This was nothing like the other rehabilitational hospitals I have had my parents in. I will definitely be using them again for my other hip! I luv the new way they are transforming the rehabilitation experience!

If I could leave a thousand star review for La Casa Via I would do it in a heartbeat. My husband and I were thrown into an unexpected whirlwind of events after his mother had an emergency leg amputation. After a series of operations she was sent into acute rehab, and after two weeks they were trying to discharge her home. My mother-in-law is 70 years old and was still on a wound vac, meaning that her amputation wasn’t fully healed and had to be connected to a machine and changed every other day. After telling our concerns about her safety to our case agent at the rehab facility, we all agreed that going into a skilled nursing home would be a good idea to give my mother-in-law time to gain her strength back.

After touring a few different skilled nursing homes we found La Casa Via. The director of the home, Camalin Jones, led us on a tour and I couldn’t have been more impressed by her depth of knowledge in the skilled nursing field, as well as her care for the people in her facility. She left no stone unturned when answering all of our questions and concerns. After weeks of stress and uncertainty I felt like she was someone I could really trust. She said she would treat my mother-in-law as if she was her own mother, and believe me, she went above and beyond to fulfill that statement.

The day before my mother-in-law’s release to La Casa Via she was turned down by the health care group, and we were informed that her health insurance would not be covering her stay in skilled nursing. We were shocked. Her apartment wasn’t ready with safety bars or furniture, she was still on the wound vac, she lives alone and my husband and I had flights back to Germany (where we live) the next day. As you can imagine we canceled our flights. We admitted her to La Casa Via on private pay because the apartment simply was not ready. While there, the director of La Casa Via helped us file an appeal to the Health insurance company, and honestly because of the incredible statements she wrote and evidence she included we miraculously won the appeal. My mother-in-law was refunded the money she had privately paid, and was able to stay until she was ready to be discharged.

Aside from the monumental assistance we received from Camalin Jones at La Casa Via during our appeal, the facility Itself was wonderful. My mother-in-law was uncomfortable sharing a room, and they were able to give her a private room at the same cost. The nurses, Physical therapists and Occupational therapists were all incredibly kind and receptive. Everyone there really took their time to sit down with us as a family. They listened to our concerns and questions and explained future procedures very clearly. They also have an unbelievably kind and competent wound-care team, that made changing my mother-in-law’s wound-vac much less stressful. My husband and I were concerned about going back home to Germany in case she was released when we weren’t there and there was no one to help her through the transition. The social worker, George Roberto and Case manager Stephanie Harmon at La Casa Via assured us that they would take care of every detail of this transition, from arranging PT’s and OT’s to making sure her medications would arrive properly and on time. And they truly did! My mother-in-law was released back home recently after we had already returned home, and the transition really couldn’t have gone more smoothly.

Though the building is currently being renovated, La Casa Via was the brightest skilled nursing facility that we visited. There are huge floor to ceiling windows in the recreation areas and each room is on ground level and has a huge sliding glass door to the outside. Since it can be challenging for patients to go outside this open and bright atmosphere really helps gives a sense of freedom to the space.

I truly can’t put into words how grateful I am to La Casa Via, their director and their incredible staff!

I am giving La Casa Via Transitional Care Center a definitive 5 stars.

My mom suffered from blood clots to her right leg which resulted in an amputation above the knee. She is 70 years old and prior to surgery had been very independent and physically fit. My mother had to go through various procedures and tests in the hospital which put my family and I in an emotional roller coaster for weeks. Our goal after the hospital was a smooth and easy transition back to her independence.

During our tours of various skilled nursing facilities in the Walnut Creek area my wife and I already felt most positive about La Casa Via after our first visit. No other tour of a skilled nursing facility impressed us as much as the one we received from the director, Camalin Jones. Her experience, dedication and promise to patient care convinced my wife and I that LCV was the right place for my mother to transition into, post hospital-rehab.

The director and the team of La Casa Via delivered on their promise 100%.

-Director Camalin Jones listened to our concerns and made sure that my mom would receive the care we were all hoping for. In addition, she went above and beyond and helped us with an appeal we had to fight against my mother's health group denial for a skilled nursing facility request. She did this out of pure commitment to her patient, knowing that my mother was not in the financial situation to pay for a skilled nursing facility out of her own pocket. Thanks to her, we won our appeal. Other points which mark the commitment of the director included: talking and listening directly to my mom (patient) on-on-one, strong consideration to the family's concern, having a good perspective/understanding of the situation, genuinely caring, great at openly communicating with the family and a strong patient advocate.
-The physical- and occupational therapists at LCV gave my mother the necessary PT/OT that she needed to regain her strength and confidence. Our primary concern was that my mom would fall behind on her therapy if she was discharged home and thus not regaining her strength back. This could lead to accidents at home (falling from a chair etc.) that could set her back further. I felt that the additional therapies she received at LCV greatly improved her strength and confidence.
-According to my mom the nurses where really friendly, timely and made sure she received what she needed (meds, food, bathroom assistance etc.). They all seemed to connect with my mom on a personal level as well and it was really nice for my wife and I to observe.
-A 'Care Conference' was scheduled between the patient, family, case manager (Staphanie Harmon), social worker (George Roberto) and therapist (Kristy) to make sure that there was an open communication concerning her health condition, timeline and plans for my mom. Since my wife and I never had to deal with this kind of situation before we had a chance to discuss any concerns we had and also had several questions answered. I also have to praise them for their warmness, openness, their sheer competence and understanding for our particular situation. They are also still keeping an open communication well after my mom's discharge.
-LCV made sure that my mom's transition back to her home was smooth and that Home Health was set up and scheduled prior to her return.
-Amazing wound-vac nurses! While in LCV my mom still had an open wound on her amputated leg which was covered by a special 'Wound-Vac' machine. LCV had a really great team of wound-nurses that made sure that the wound was well attended.

In short: LCV delivered on their promise and commitment to patient care. They really provided us with the feeling of relief and that my mother was in good hands. She received the necessary warmth and care she deserves. Administratively things were professionally handled and there were no surprises.

The patient care is horrible. There are patients that are falling out of there beds and locking themselves in the bathrooms. The patients are not being cared for in a safe manner. The patients beds should have bed rails for their safety.

My grandmother was initially sent here after a stroke. It is an awful place. When I was visiting, she pushed the button to summon the nurse for pain medication, and the nurse took almost 45 minutes to come to her room. The staff was not warm, and the facility was incredibly depressing.
