Lone Tree Post Acute, Antioch CA

Love working alongside all the staff at this facility! Everyone is so kind & efficient and truly care about each and every patient’s well-being:) I am very fortunate to be working here!

My dad has been well taken care of at Lone Tree Post Acute. Every time I visit him, he always talks about how good the food is. The staff make sure that all of his health and hygiene needs are met. The physical therapy team is awesome! The staff have done everything they can to make sure my dad is comfortable and happy.

I was at the facility visiting my grandma today and felt compelled to write this because other family members who were looking up the address to visit, saw some of the other reviews. So here’s my take on everything. This isn’t our first rodeo unfortunately. This is her THIRD yes THIRD time coming here and I tell them when we leave we hope to not see them anytime soon, yet here we are again, another fall, another hospital and another stay in a nursing facility. My first time having to pick a place it was overwhelming, and scary. How do you pick a “nursing home” when you hear such terrible things about these types of places on the news. I pictured a dark building with sad, scary patients & all the while everyone you know is like “oh yeah my grandpa died in one blah blah”. I am my grandma’s person, the one who helps her shop, go to appointments and checks in on her daily. This choice meant a lot to me, because where is better than being home, but the hospital advised me (much to my push back) that I was not going to be able to care for her in the state she was in. So I caved and started the process of finding her somewhere to help her get better, so we could go back to hopefully our normal life. I’m not going to lie I dragged my feet thinking the hospital would give me another day to choose but they said she was ready and her insurance said it was time for her to leave the hospital (ugh whole other thing but whatever! I won’t ramble on that one). It was right across the street from the hospital so I walked over and they were able to accommodate me for a tour immediately, first good thing! I explained how uncomfortable I was (i may have even cried) and right from the start the head nurse gave me their number and said please call me if you need anything, which I hope they don’t regret doing now ha! After touring and talking to someone in the admissions, knowing I didn’t have much time and this was the closest to my home in discovery bay, I decided to give them a shot. First night was tough on all of us, I think she called me every hour until she fell asleep but I don’t think that had anything to do with the facility and everything to do with not getting why she couldn’t just come home. But of course my guilt started settling in on if I made the right choice. I was there first thing the next morning and there was a therapy team in there already seeing what she was currently able to do, which sadly wasn’t much. Then they called me for what they called a “care conference” and I got to meet everyone who I guess is on her care team. We talked about foods she doesn’t like, what type of support we would need when she comes back and what type of activities we could try and encourage her to participate in to get out quicker. She met lots of people there, when new patients would come in and then others would be able to eventually go home, some we liked more than others but really we were all in the same spot, needing help with care. When the day finally arrived that she was ready to go home, I think we were both a little sad to say goodbye to everyone, but we did because we were even more excited for her to come home (and i was excited to not have to make that drive all the time!). Sadly 6 months had passed and she fell again. Back to the ED we went, but I felt more prepared this time for all the things they were going to tell me that needed to happen next, she needs therapy, she needs care blah. Hospital called the facility and back we went for rehab again, much less scary this time! They knew me by name when I arrived to visit this go around and all together it just felt easier. I don’t think the hospital paints the right picture, I thought this was just basically a mini hospital, but really what it was, is a middle ground to going home. She stayed that time around 4weeks and came back home and we were back to our lives again. But here we are yet again, stupid sidewalks always messing up our routine! Thank you Lone Tree for being here for our family and our grandma, hopefully her spitfire moments we love her for, you guys do to!

My 90 yo dad was just admitted yesterday following hospitalization for a fall. He typically lives at home with my mom, and takes care of his home and finances.
He used his call light at 1: 00 to request help to the commode. After not getting a response, my mom asked staff to help him and was told they would let the nurse know. No one helped until 3: 00 (by then he was unable to hold his BM). They cleaned him and left so quickly he didn’t have time to tell them that he had to urinate. My dad is left embarrassed and upset!
This is elder abuse! Don’t send your loved ones here!
I would give zero stars if possible!

I have experienced the highest level of care from a dedicated highly inspirational staff. Every touch point has been met with high hospitality dignity and respect. I feel most blessed to have experience compassionate Rehab and care here! I am blessed and better because of the staff and their commitment to serve others.

My Mom was in the Lone Tree Facility for 2 1/2 weeks after a fall/covid and we were so pleased with the care team. Physical therapy was done every day (many times 2x/day) and she progressed so much better than she had at a previous facility (in Rossmoor). The wound care nurse was also great (Piper?). The building may not look as nice as the Rossmoor location, but the people were fabulous at Lone Tree and that is truly what made the difference in her coming home in great shape.

THIS PLACE IS THE WORST! THE NURSE PHILLIP THAT WORKS NIGHT SHIFTS WAS JUST HORRIBLE! They treat their employees sooo horrible and they do not care about their patients! If I can give ZERO STARS I so would!
