A very nice place, the nurses are very responsible with their work, they have a lot of patience, especially with the elderly, the food is very good. Thanks to everyone, especially nurse Vicktoria Hernandez.

My mother's stay went terrible, she was having a heart attack and was calling for a nurse and nobody came in. She called my brother who called the facility and told them are mother was having issues and to go check on her. My brother drove there and when he got there nobody had still gone in to check on her. He asked them to call 911 and they tried to talk him out of it. She had open heart surgery yesterday!

Great speech, physical and occupational therapy. Very nice staff. Great Director and Admissions personnel. Safe, clean and patient focused for short term SNF.

We admitted a family member approximately 4 months ago and was recently discharged to an assisted living community.
From the moment we arrived to Pavillion at Sunny Hills, we were greeted with kindness and assurance that our family member would be treated with great care.and to the day of discharge this was completely accurate.
From the amazing PT/OT staff that worked hard to accomplish our goal, to the administration. Marianna and Joanna, to the cleaning crew (especially Leticia who constantly had a smile on her face and brought joy every day). We are grateful!
By far though, a special "shout out" goes to Elena. Never have I seen a more consistent, dedicated and compassionate worker than Elena. Daily she greeted our family with the same work ethic and complete knowledge of her job as well as the same friendly attitude that genuinely resonated to our family. Elena always made us feel safe and confident in choosing The Pavillion. If I could, I would hire Elena TODAY!
Others that showed "over the top"care and pride in their job was Alejandra, Dan, Joy-OT, Sarah PT, Daisy PT and Jacob.
My family is grateful for the care we received and cannot thank the staff enough. Grateful.

I am unhappy with the time lapse of attending to the patient. Some of the staff passed off obligations of care to other staff members. I was there visiting everyday. I understand the staff shortage but it is their job to make sure a patient is tended to in a timely matter.

Elena Bello cared for my father during his stay at the Pavilion. She was very kind to my father. Although my father didn’t speak English fluently Elena made an effort to communicate with him and made him feel comfortable. He would share with me that Elena was good to him and would communicate to her his gratitude and approval by giving her a thumbs up when she came by to do routine checks during her shift. Elena stood out to our family as someone who truly cared for her patients. My father has since passed away. When Elena heard that my father passed away she came to the funeral to console our family. Pavilion is blessed to have Elena on staff.

On an early Friday morning, I unfortunately fell on my right side fracturing my hip. St. Jude Providence in Fullerton confirmed I had a broken hip bone.

Saturday morning I was operated on and sent to the Pavilion’s at Sunny Hills across the street from the hospital.

In the beginning all was terrific, each day I was making progress. With the assistance of my physical therapist, I was able to walk, transfer from my wheelchair to my bed and restroom.

On one morning a week later, my therapist was assisting me and I fell. Unfortunately my therapist did not catch me and I dislocated my injured hip. I was sent back to St. Jude Hospital to have my hip placed back into the socket. (Painful)

I then returned to The Pavilion’s to recover and start over again. At 91 years of age, recovering from the Anesthesia was difficult. My injured leg was attached to a foam wedge to keep it from moving and in position.

Your not going to believe this… Four days later while receiving a visit from my daughter-in-law and my granddaughter, two aids came into my room to assist with lunch. The two ladies grabbed my torso and yanked me upward, I screamed as my hip was dislocated again.

I again was transferred to the hospital, put under general anesthesia, and had my hip put back in place. My third time back at the Pavilion’s to recover did not go well. My body was not recovering from the anesthesia, was heavily medicated, and was unable to eat.

Not long after, I was sent back to St. Jude Hospital for a UTI, a colon infection, and had lost a great deal of weight. My doctors were extremely disappointed with the care I had received and placed my on hospice care.

Sadly, three days later I passed away at 1: 30pm in my room at St. Jude Hospital.

As I look back at my stay at the Pavilion’s, it appeared as if the organization was understaffed, many of the nurses aids were not informed of my injury or why I was even there. My son was told by the Pavilions management how sorry they were for the unfortunate recovery experience. The manager started there staff would be receiving additional training and have learned from their mistakes.

I hope my death was a walk up call for the facility and may no one else have to go through the same experience.

I’m very pleased with how my 86 YO Mother was cared for at the Pavilion.
This is a very friendly staff and patient, even when a patient is being difficult.
I cannot give praise enough to the Physical Training personnel. They are very capable and have excellent people skills. The PT training room is excellent.
The facility is clean and inviting, spacious and hospitable for visitors.
I gave a 4 vs 5 rating only because the food could use some improvement and response time can be a bit long sometimes, but I am very pleased with the quality of the staff and facilities.

My 86 yr old Mom entered the facility in May and had a 3 week stay. She broke her pelvic bone and required no surgery, but needed 100% assistance.
I read reviews, some great, some not so much. Well, I can confidently say that her stay at the Pavillion was very good. Look, all places today are understaffed, and we went in expecting some delay in some services like reaction time for changing diapers…Mom complained a little but understood and it was never that bad.
The staff are ALL exceptionally kind and helpful. The PT staff are so good at what they do. They really got my Mom to push through the discomfort and didn’t let her rule the session, LOL. My siblings and I were there with her trading off every day so we spent many hours observing the place.
The food is not so great, but seriously what do you expect. …. And I felt there were too many sweet items on the tray, sometimes. (she had no food restrictions at this time.) She did love a few of the dishes, and you can make changes and choose other options. Or even order in.
All in all, we were pleased with the facility and would recommend it for your loved ones.

The patient has told me that the CNA who was assigned to his room brought some trainees in one day and he was very inappropriate in. Touching and handling the patient in an inappropriate manner. Showed little to no respect for the patient's privacy.
He also made very inappropriate comments to the training group about the patient. Also during his transport the patient's ice machine was either not loaded into the transport vehicle or somebody kept it because he never had it and we had to Get another one. Other than that everything was satisfactory.

The one who cleaned the floor to food service to personal care staff. I have no complaints at all. The Marianna's we also timely and competent. I have no complaints at all.

I cannot say enough about the good care that I got here. Every single person from nursing to PT and OT to the aids, dietary social work case management they all went above and beyond. The care was spectacular. The rooms were great. The food was good and I have lots of food restrictions and my CNA Elena went above and beyond to make sure that every single time my food was correct. She found out what I like to make sure I had extras of it. She made extra time to make sure I had a shower on my day of discharge, which she did not need to do. Really the care was extraordinary. If you need to go anywhere for rehab, this is the place. The physical therapist and occupational therapist listened to the challenges. I have getting in and out of my house and assisted with good strategies. They arranged for a pain management consult for me and I saw Dr Sayer and they were both extraordinary. Again, if I could give it 10 stars, I would.
