My uncle has been there for 6 years and has had wonderful care. A shout out to Brenda in Social Services for helping me this week. She was very helpful and got right on my concern,

My father was admitted Sept 1 for post rehab of a broken ankle. He went in a healthy 230lbs and was discharged Oct 26 on Hospice at 150lbs and died 4 days later! I have too much to write here but he was neglected beyond belief and let down by the ENTIRE facility and medical staff. I was there every day and almost every day I complained to nursing and anyone who would listen that I was concerned about my dad and his lack of eating and drinking. First month was "ok" despite me having to clean him EVERY day and a host of other issues. He was the sweetest man and never wanted to "bother" anyone and had nothing but compliments to every person that walked in his room. After his medication was DOUBLED, he went downhill fast and stopped wanting to do rehab and just wanted to sleep and not drink or eat. I asked nursing EVERY day if they gave him something to make him sleepy and expressed my concerns to the head of nursing several times and she said she would have Psych and the doctor look at him. She was too lazy to get off her a** and go see him. We were talking about bringing him home and the Administrator said "if it were my dad, I'd bring him home on Hospice". I said "He's not dying! " She said "no no, he can graduate off Hospice in 6 months, but they will provide nursing, beds and aids. "He will perk up being around your mom and they will reevaluate him in 6 months and he can graduate out of Hospice". This sounded great and we brought him home. He died 4 days later. This sweet man needed a hospital, NOT Hospice! The only time this facility contacted me was if it was a billing or insurance issue. Never once did they speak to me about my dads health. I am finding out about his meds and evaluations etc only now after I requested copies of his records.
You do not, nor should you ever, die from a broken ankle. My dad survived the surgery, 11 days in the hospital and the first month in rehab just fine. It was only after the medication change that he went downhill and was never monitored closely and my cries for help fell on deaf ears. Shame on me and I will live with this the rest of my life, but SHAME on every single person in that facility that neglected my dad and allowed him to go from 230lbs to 150lbs (in 2 months) and let him die.

Got here today @ 3: 30, they still haven't registered me. Which means no pain killers. My roommate told me she has to beg for her pain meds all the time. She has been her almost 2 weeks for rehab. Same reason I am here.broke my ankle in 4 places and sprain the other.

I am willing to update this review, if they prove to be different than the impression I am getting so fat

My Mom was place in Bradenton Health care for post surgery rehab. The place was not very clean. Was tremendously understaffed and non responsive to call buttons and to the needs of my Mom. I live out of state and tried to get in contact with the facility only got through a couple of times and was NEVER had a return call. One of the times I did get through and asked for Crystal again, she was there by the station told the person that answered the phone she was busy. Crystal don’t bother calling Mom is now out of there.
She was discharged and sent home where she lives alone and cannot tend to her needs because she wasn’t finished PT. She was sent home without any home care. I am saddened and outraged that you Dumped her to free up a bed for you. I would not even send my Dog there. My dog gets better care in the kennel.

My five stars are for Crystal Oliver. I was very discouraged with some issues that were going on with my fathers care. Crystal listened and was very professional during our discussion. I felt heard and felt very assured that the issues would be taken care of. I felt she went over and beyond in dealing with the issue that I was concerned with. She is a great member of your team.

The facility is not what I wanted it to be unfortunately my mother got a horrible rash however Crystal Oliver the social services director was the only saving grace.she was intelligent, informed, and had unbelievable knowledge of policy and insurance practices and state law.she set us up for complete success with tons of home care PT, OT, skilled nursing, home health care, meals on wheels.we could not have brought Momma home without MS. OLIVERS expertise and effort. She's goes above and beyond protocol and rates a 10 with our family.
