My dad has been there for 2 weeks. It is the dirtiest, most un sanitary facility I've ever seen. The courtyard has weeds 12" tall, moldy, torn and rusty chairs for patients to sit in and the most shocking is the facility passes out cigarettes to patients. The walls are filthy and moldy and the AC filters are also black. The condition of the place is truly unimaginable unless you see it for yourself.

ROTTEN ROTTEN ROTTEN! People, please do your research on this terrible facility BEFORE you or a loved one goes into this place! Myself and family learned the hard way, my father who was there 3 weeks was considered a fall risk, he happened to fall out of his bed, yep, NO guard rails on his hed. They will steal your things, they belittle, laugh, make fun, keep things away from, if you need dialyis, you'll be waiting, a hour or more for the bus to pick up. You need to be washed up, good luck, they let ya sit in it and be degraded. Absolutely horrible facility! Please, think twice!

Would not even take the dog there, In humane is an understatement. No showers for up to two weeks. No diaper changes for hours, no visual inspection for hours, missed meals.
Poop on the bathroom floor for a week at a time. False medical study’s. No dentures till afternoon. Just a total inhumane place. Complained to the highest level of administration and staff and all the do is blow smoke.

My 90 year-old father was transferred after a short hospitalization, for short-term rehabilitation. I live in VA. My brother was down from VA staying with my 91 year-old mom in Ft Myers in the aftermath of Ian. As a former rehab nurse and case manager, I have some idea of what should be expected in rehabilitation. My dad was not seen by a therapist (PT) until day 3 of admission. We still have no idea what other therapies saw him (ST or OT). We had a phone meeting with SW on day 2 of admission, but no communication beyond that. Messages left for them went unanswered. Another team meeting was scheduled for today by phone. The time came and went. My brother called, and was told there was an emergency with a resident needing to be transferred to the hospital, and their conference room was tied up. They wanted to reschedule for next Thursday! My brother said that was it, have dad ready, he was driving up and getting him. As it stands, we have no clue what therapies have recommended post-discharge. This is appalling to me as a rehab nurse. Never again!

The charge nurse Grace sits and laughs at these patients while they beg her for their smoke break. 5 CNAs all sitting behind that desk not helping not a single patient. They leave these patients food trays out in the hall for 10-15 minutes before giving them out so they're cold. Do better these are our families. You all will be reported to every news channel.

The charge nurse Grace is absolutely a terrible person! I seen a video of her laughing at senior citizens while she made them wait over an hour for a cigarette break! Then threatened not to give one at all if they complained! She is a terrible nurse and doesn't treat people with respect. I would not send anyone here! Come on Magnolia, you can do better!
