This facility is the worst facility I have ever been to. My dad is currently there for rehab. He has been neglecting since day one. He has been there since 12/26 and to this day has not been offered or given the supplies (wash clothes/ towels/ soap) to bath. They never gave him a toothbrush/ toothpaste. He never has sheets on his bed when I arrive. The room is dirty, shoes stick to the floor when you walk on it, there is dirt and small debris in and around the bed, there is an odor of urine the minute you step into the hallway leading you to the rooms. Every time I go I spend the first 5 minutes making his bed, disinfecting the side table, gathering the full garbage bags and taking out the trash. The nurses and physicians have neglected him. There is no healthcare at all. This place is committing medicare fraud big time. The call light stays on for one to two hours to the point that I have to call corporate and get them to contact one of the nurse supervisors that lives near and she has to drive up to the facility to have the staff provide care due to no one answers the phone after 5pm. This has happened more then once. Dylan in corporate definitely does a good job of telling me what I want to hear and then doing absolutely nothing to have the situation changed. Due to the holidays I have not been able to report them however tomorrow I will be calling the ombudsman, Medicare, Advent Health (they transferred my dad here) to make a formal complaint. I will also be filing a grievance. My dad has requested to talk to a dr since day one and was told not until next week on Tuesday after the holidays. We all know Dr are on call 24/7. He is a chronic pain patient and had to beg for pain medication. I had to get involved for this to happen. The nurses/cna are rude, unprofessional and sloppy. They lie repeatedly. I am a nurse of 19 years and would be embarrassed to provide the level of care this place does. These employees belong working in a prison or not in healthcare at all. My dad being in this facility has caused me extreme anxiety to the point I am unable to sleep at night. I am doing everything in my power to get him out of there.

I worked there through agency for many years off and on. I recently worked there for about 2 months. People are being abused and neglected. Sometimes the people who are needing help getting fed.don't get fed. The director KAY has a theft ring going on amongst the cnas.and if you speak up you get fired. They let a trachea patient lay in soil on south wing flushed properly. Jerica Greene is guttervillle and so is Stacy Blue.they are bullies. Another thing they do is swap medications amongst patients. They also kept 1500 of my pay. Ive hired an attorney and reached out to state and the labor board for my wages.the trachea family
complained but there's nowhere else to put his mom. Another patient needed emergency surgery after leaving there because his peg tube was never tended to. They allow patients to get alcohol delivered to a point they are falling down drunk and needing ems. This place is a license in jeopardy kind of place.
Kay jerica and were all hired within the last year. The place has really turned into hell for families and patients. And Josh the administration guy will promise families the world just for a check.

This facility needs to be shut down. The patients are suffering. They can not be cared for properly. They give 22-25 patients to only one CNA which is ILLEGAL. Many employees complain but are continuously ignored. The staff is suffering. This place is horrible.

My grandmother was here and died from sepsis from their filthy careless facility in September. I don’t know how these people sleep at night.

My great grandmother was there for 2 years and by far the worst place ever. She was very malnourished when she was transferred and when asked about it they said that she refused to eat; which is very untrue. She also had various lesions and bruises on her body. This place should be shut down ASAP!

OMG. Can’t believe this place is open and take care of patients. This place is horrible. My mother was in there for 2 nights and I got her out of this hell hole. Place is dirty and smells like piss.my mothers room had dirty glasses from other patients that were left over when they left and a IV bag still hanging with fluid still in it. Most of the staff were rude. This place is more like a homeless shelter with patients hanging outside all day smoking and hanging out in the hall way. Do not I repeat Do not send your love ones here. The State should close this place.

Absolutely horrible. Please don’t take your family there! Only 2 people cared about my sister. And that was the hard workers in there Rehab section! Thank God for them.

This place is trash! DO NOT WORK HERE! DO NOT BRING YOUR PARENTS HERE! They need more help and more people to work! THE DON IS THE WORST AVANTE HAS SEEN. She got kicked out of 3 facilities in the past! She has no respect. She’s rude and is only about the money. She steals from facilities and has an intimidating personality. They have to get her out of thereAnd instead of putting more people on the schedule they are giving the aides 17 people a piece to take care of each shift! And expect quality care? It’s ridiculous! They must have many incompetent people running the place. Which is not surprising! It’s show.

My Mother has been at Avante for 1 1/2 years so far. They take good care of my dear Mother. She is doing well and the staff there is friendly and kind to my Mother. A little bit of updating and renovating wouldn't hurt of course but over all my Mother is happy and that is what is important. Thank you Avante

Horrible place, as most are, however, this place takes it to a whole new level. Most nurses/CNAs don't care AT ALL! I found ants crawling on my friend 2x so far. This place sucks! STAY FAR AWAY. Zero stars!

By far the worst place ever. If I can give less than 1 star, I would. If you care for your loved one, do not take them to this place. Today I found my mom's concentrator (breathing machine) completely turned off. Someone physically went in her room and turned the machine off. This is neglecte to death. Unfortunately she has been there for a few months and it only gets worse. I have spoken to upper management many many times, they make empty promises and never follow through. They have changed their director of nursing 3 times and seems to only get worse. I would gladly take her to a Different facility but unfortunately they are the only facility that would take someone in her condition. They don't even do the basics as the state requires them such as repositioning every 2 hours, oral care, tracheostomy care and much more. Many times, us, the family would do all of that and more ourselves because the staff does not. Their equipments are not even up to state code of being calibrated each year like they should be. Again, stay away from this place.

Horrific. Our mother was moved her for rehab after Covid treatment in the hospital. There was one RN that was kind and helped me wash her. I had to use soap from the hospital and she could not get anyone to bring her a wash cloth. Mom is mobile with a walker. They were surprised I brought her walker. Mom wears Depends in case of accidents, but was told to use the bathroom in them by a CNA standing right next to her. I work in inpatient care and would not hire one of the CNAs I met there. The building is old and run down and the was a bed with wet sheets in the hall. PPE was provided with no instruction. I know how to use PPE due to my work training. There was a roach in the shower, the shower that backed up. To top this all off, her new red walker was switch by removing her bag and luggage Tage to and old black walker. All of this happened in less than 24 hours. The activities coordinator called me to tell me the Mom could participate in group activities after telling me she was in isolation. No rehab. Mom was left in bed for the entire time. I did check AHCA site before allowing her transfer. I made a mistake sending her here.

If you do deliveries for food, be aware that the receptionist will cancel the order before you can complete it thinking that you won't get payment at all. Can only wonder what she's doing behind the scenes.

Poor management! They do not care about the residents or staff. It is a revolving door for DON's, ADON's, CNA's and NURSING staff. Staff are stressed out all the time and short staffed working doubles. All management cares about are heads in beds. They love to silent their residents!

This place is the absolute worst ever! Google seriously needs to consider allowing zero stars. I went to visit my grandmother who I had not seen in a couple years. She had been here for about a year, when she arrived here she walked in and could speak, but when I got to see her, she could barely speak and she certainly could not walk. Her leg muscles looked atrophied, like she got little to no exercise or movement. She was diagnosed with dementia, and that can affect your speech, but why can’t she walk? We met with Heather LaFrance, Nicole Sherwin, Deb Trotter (who was her nurse that day), Beatrice Ferlu (social services) Debra Davis and Trisha Kruse to come up with a better care plan, and reduce her meds so she is not out of it all day and night. Only to find out none of it was implemented except for the reduction in dosage for her meds. We were also concerned that she wasn’t being cleaned properly because in addition to her being soiled when we arrived she also had two bed sores. They told us that it was her bath day and that they would bathe her, although it had not happened while we were visiting. I had to come back that evening to make sure they gave her a bath, which they had not! I had to tell them everything that I was told earlier that day to get them to agree to bathe her. Night shift blamed it on day shift! It was a mess! The workers seem over extended and the facility is short staffed. There are 3 to 4 patients to a room! A normal sized two person hospital room! We asked the evening nurse when was the last time she had a bath and the attending nurse told us, she had not had a bath all 2023 - she could not find any records for baths although they are required to document them. I had to visit her twice a day while I was there to make sure they were even changing her. The first 3 visits she had a soiled diaper and urine was coming through her clothes. Later we found out she had a fractured rib, fractured femur, c-diff (for the 2nd time), and several UTI’s. We only found this out because she had to be rushed to the ER for being non-responsive. Thankfully, the ER would not allow her to be release back to this facility. Please do not send your family member here and if they are already here and are not getting the proper care please report this place to the state. They need not be in business if they aren’t willing to do their jobs! They will have to give an account for how they are treating our elders!

I have had my Mother, J. K, at Avante in Ormond Beach for the last 6 weeks.
While it was a difficult decision to place my Mother in a Long Term Care Facility I have ZERO regrets / guilt having chosen Avante.
As a Nurse / Nurse Practitioner in Ormond Beach / Daytona Beach for 20 years I hear EVERYTHING about all of the local LTC facilities.
Not one has a good reputation.
But I had to choose.and in a short period of time.
My first introduction to Avante was David Carter. Then I met Wendy. No words can express my appreciation for their support at the time of admission of my Mother.
From there I have met some of the hardest-working Nursing staff anyone will meet. I can't say enough about the staff.
So.to the critics out there. Avante is a Long Term Care Facility providing Nursing Care. They provide medications, rehab, incontinence care and basic bathing/cleanliness to name a few of the Long term skills offered.
It's not a hotel / resort. If you have that expectation then you will be rightfully disappointed.
In general, end of life issues don't involve resort-style amenities. My Mother, at the end of her life, as with many in her situation require being clean, fed and engaged with staff.
All of which are achieved
At Avante.
My Mother's medical situation (and other residents) is not the fault of Avante. Instead, I look to their staff for compassion and basic needs in response to her current situation.
The staff has delivered beyond my expectations.
I highly recommend Avante of Ormond Beach for caring, efficient and decent nursing care for your loved one.

Excellent staff food is good rooms are clean the best physical therapy team occupational therapist I have nothing negative to report about this facility they have helped me to get out of my chair and walk again thank you everyone for your help at avante John Haney Holly Hill Florida

My mother has been at Avante for 24 hours and it has not been a great experience. My mother has Alzheimers not to mention her many other medical issues. She fell in a state of agitation and panic and broke her ankle 3 ways with dislocation. She had surgery with many plates and screws and is non weight bearing for 6 weeks which is why she is at Avante I get she is in Rehab for her ankle but can she not even expect her ongoing medication since they don't want us to bring it? Can she not be made comfortable, can she not be safe? If we had not been there all day today she would be sitting there at risk of falling. We cannot spend the night there but nobody can let us know when she can get medication if she can get rails for her bed there are no chairs anywhere to sit in there is no place in in the room that if she is in too sick if there were chairs let alone spend the night with her so right now she is there scared agitated at risk of trying to get out of the bed to try to find someplace safe to go and there is no medicine after 24 hours there is no extra pillow there is nothing for pain there is nothing being done to help her after only 24 hours. I cannot imagine how the patients without family get through their time there.

We were able to get my mother out of there and home after 11 days. She has regressed alot cognitively but she was much happier. She is back in the hospital with a raging super bug due to cross contamination in her urine, her foley was placed in at Avante the day before she came home and it was a circus getting it in, broken bed, the nurse couldn't see the urethra and inserted it in vagina before finally getting it in place. I don't know if this started the cross contamination but I think it is.

My mother never recovered from the bacterial infections and was in the hospital and came home to go back a day later when she died.

If I could have selected zero stars for a rating I would have, but to even give a review I had to select at least 1. My sister-in-law is at Avante for rehab after surgery for a broken hip. I live 1100 miles away and can’t get there soon enough to rescue her! There has been NO rehab therapy! She calls for meds and gets no response for 6 to 9 hours! I called to talk to the director of nursing several times and get no response. Tried calling her wings nurses station and no answer! Can’t talk to a staff member, just the receptionist who pages and still gets no results! This place should be inspected for maltreatment for sure! She has had to walk to the restroom with a Walker by herself which is forbidden on her hospital prescription! DO NOT LET YOUR LOVED ONE BE ADMITTED HERE!

My mother was recently a patient at Advante for therapy for a broken hip. The 10 days that my mom was in the facility she was not sponge bathed once and was left to sit in soiled diapers sometimes for several hours. When the nursing staff was questioned myself and my wife were told by several nurse aides that they were short handed and could not be at her becon call. After I voiced my concerns to the director of nursing and the faculty administrator the behavior of the nurses and aides improved, but I can honestly say the therapy staff generally cared for their patients and improving their well-being. If my mother needed another rehab facility I would think twice before sending her back to avante.
