Last Wednesday we found my Mom in respiratory distress, laying off the bed. Could not get help. Everyone said they would get someone.no one. After 90 minutes, we requested she go to hospital. I called front desk and said I would call 9-1-1! The staff wanted her to stay and give breathing treatments. No stethoscope or oxygen mask.nothing offered! She went to hospital and was intubated on the way there!

She was in hospital previous week from their care as well for severe impaction on tube feeding! Took 3 days of meds! Filing investigation!

Nearly killed my grandma and left her off her oxygen for hours if my family wouldn't have shown up she would have passed away due to their negligence. This place needs to be shut down. My grandma is still in the ICU because of their care. Or lack of it. Never put your family in this place. 0 stars.

This place does not have enough staff to care for your loved ones. Your family will not be showered. The admin.will not speak with you. They keep putting oyu off. I have called state on them several times. They love to say they are short staffed. The food is awful. The place smells bad. You have a different nurse daily because the use agency instead of hiring their on staff.

I personally do not have a loved one in this facility, but I have had several business dealings with the staff and residents here. The staff has always been top notch, helpful and very nice. The residents always seem happy and well cared for. I would suggest that anyone that is looking for a place for your elderly loved one to go, this is the place.

The managers in this place suck I know multiple people that work here and they don’t take care of there employees, they told my friend that they would fire her if she didn’t come in and work knowing that she has had Covid the boss there still demanded that she came in I think the state needs to come and fix a couple of things in this place

My mom works here and I’ve had lots of family members that were residents here. The staff and employees are great. Kizzy is awesome at her
job and makes everyone feel welcomed and comfortable. She can answer any and every question you have and if she don’t know, she will find out. Emily is a great administrator and loves and treats all the residents just like family. I would recommend facility to anyone and everyone.

My name is Carolyn Williams,
My dad was a patient on the Rehabiliation unit. Vizell Robbins, Jr. March 2-April 27 2022.

I am writing you to tell you about the outstanding group of Nurses, nurses assistants and entire care teams that cared for my dad during his stay there.

Everyone that provided care for my dad was kind, caring, considerate, and understanding.

They all went over and above to make sure my dad was comfortable and turned as close to every two hours as possible,

I appreciate the physician assistant and social worker that came by for a meeting to inform us of my dad’s declining condition, of which I could see, but most of my siblings could not, so I was grateful for their kind intervention!

A day before he transitioned, your sweet nurse Pat came by to let me know my dad was declining quickly and gave me a hug, words of comfort and support checking on me frequently even though she was having a very busy day.

I hate to start naming names because they were all so very wonderful and gave me great comfort, I cannot remember every ones name I wish I had written them down, I hate to leave a name off, but I remember Rebecca she took special care to learn how he like his coffee and made sure he got his nephro down, and just checked on us to make sure we did not need anything.
Chelsey and her partner came in through out the night and turned him and lotioned his back about an hour before he took his last breath. So I know he was comfortable just before he passed.

Joanie, I’ve known many years was a great comfort and my go to Tina, Dot, Annette, Tan and others I’ve also known were very supportive and bought us great support and comfort.and many others, please forgive me as I can’t remember each one’s name!

Your administrative and office staff were so helpful and kind
Kizzy, Debbie each and everyone, Tee and Benny, your therapist, kitchen staff, chaplain all went above and beyond to assist my dad and myself to provide excellent care and comfort in our time of need and distress. Let me not forget your maintenance team and housekeeping they were also kind and helpful.

Your in-house direct dialysis team were awesome as well. I’m so grateful that you have this capability in your facility, this was our major motivation for seeking care here as my dads condition would not allow him to travel to any out patient facility for treatment. This was truly a blessing, had we known we would have gotten him transferred earlier to your facility.

My dad was truly a kind loving man, he never met a stranger he always had a smile on his face and a kind and caring word to all he met, he treated everyone with kindness and respect, I miss him more than any one would ever know.

Special thanks to Lori and Debbie who worked so tirelessly to get him admitted to your facility. I must add that one day I was very distressed, overwhelmed and tearful Debbie took time to stop and give me a hug and words of encouragement.

And lastly but not least on the morning of my dads passing as we were waiting for my dad to leave with the undertakers a young man who was not on my dads unit
Stopped by to hug each and every one of us (his aunts) my dad and mom had helped raise him as foster parents - I remembered him over the years and pointed out to him and my family who he is- he was so sweet and he thanked my siblings for the care and guidance that was given to him when he was just a toddler. It made us so proud to see that he has grown into a handsome productive, positive, kind, blessed young man.

I know this note is long.
But I have heartfelt gratitude to your staff for the kindness shown to my dad, my family and myself. You all are a blessing to your patients and our community.

Thank you for your time. May God continue to bless Chautauqua Rehabiliation and Nursing facility, you and all your great team of employees.

I have worked 34 years here at CRNC. It has always been a great place to work. I have had many educational experiences here and opportunities for growth. I believe our facility goes over and above to help not only its employees but our residents and their families as well.

This is a awesome place to work I've been here 12 years and enjoy everyday. The staff is great and the envierment is so welcoming. I refer everyone to come stay and work or just come for a visit.

I am pleased to recommend this nursing center to anyone. When my mother in love was in there they took good care of her. They made her feel comfortable when she was working with the physical. We would like to thank Kizzy Tyson and her staff.

Some of the staff are rude. Breakfast is served at 10 in the morning. I would not put a family member in the facility again. When he finaly received breakfast it was cold. Most of the meals are cold. This facility gave much better care 36 years ago when i worked here.

I have worked at Chautauqua for almost 11 years and this place has been great! I have worked many different shifts in this facility and everyone works their best to take care of our residents. The residents are wonderful and the staff do everything they can to help make our residents lives more enjoyable.

One of the best buildings I’ve been too. The cleanliness of the building is spatular! The slightest inconvenience the staff are on top of the problem as quickly as they can be. I would definitely recommend to anyone wether they need therapy or to live. Absolutely amazing ️

The staff upfront are very rude. They act like they have to ask a million questions and be in your business. The dietary assistant manager is very rude. She thinks she has the right to hold stuff over people's head. I was out for 10 days and I came back that's all I heard is ur were out n this n that. And kept holding over my head I was out and other people's head when there out. I will never work at rude facility like this one. I'm sure they would have alot more people work for them if the staff wasn't rude.
