This place is a beautiful mess. Mom was admitted on 12/19, never bathed, missed lab work due to admin error. (per the staff). On Wednesday 12/27 had a welcome meeting w/staff. Was told my mom was doing great, she could walk 400 feet (my moms BP drops sitting or standing causing dizziness). I was told she would be discharged on 1/2/24. On several visits I came to find my mom soaked in urine, clothes wet to hear a cna tell her she not that wet. Her bed sheets were soaking wet as well. By Saturday 12/30 my mom had rapidly declined. I noticed the administrator Shawn and asked to speak to him. He could have cared less about my concerns. I told him if he could get someone to bring my mom downstairs I would take her to an ER myself. I said something was wrong. He said he would have a supervisor come speak to me. The RN found labs on my mom and called the dr.who stated he should have known about my moms labs 2 days prior. He ordered IV fluids and ran stat labs. On 12/31. I was called and informed that the Dr.decided to have her sent back to the hospital bc her labs had gotten worse. Once admitted to Baptist, she still had on the arm band from her admission to Baptist prior to her getting admitted here. The team at Baptist stated they did not bath her at all. My mom was pending discharge 1/2 because per the care team here she was doing so great. I have asked to speak to the DON since Wednesday 12/27. I still have not heard anything. If I had not forced the issue on Saturday for a Dr.to be called about my moms labs they would have discharged her this week, I told the Administrator on Saturday that even with the pending discharge, I was taking her straight to the ER once they discharged my mom. I was also told they try to keep a certain population there which means that some people can stay a long time but people of color cannot. They lie and say minorities are doing well in order to get them out of the facility. I would not recommend ths place at all. Beautiful mess.

The nurses, case worker Amy, Case worker Courtney, Cas manager Laura and all the staff were Awesome. They really take Great care of the patients by making them feel right at home. Since going through this process with my brother, they have been the only staff that has came through and taken the time to walk us through all of this. Highly recommend there facility to all. Great Job to the Dolphin Team.

If your reading this searching for a place for you or your loved ones. I promise THIS IS THE PLACE TO BE. My Dad (last name Coleman) was here in room 2117 until today and he was so comfortable, happy, well fed, well taken care of, nurtured, and much more. I could go on for days about the excellent care and place they have here. The whole place is kept so clean everytime. Staff is always friendly and warm. My Dad was so happy here. I am beyond pleased and wished he could have stayed long term eventually when the time comes. I am very grateful for them. I was in Shands TCU and let me tell you it was a terrible experience for me from the staff care the environment and etc but this place I have not one I mean not even a teeny tiny complaint about. It really gives above and beyond beautiful care in a warm comfortable clean comfy home like environment. I highly recommend and will forever be grateful for the staff here. They kept me and Dad in the loop always as what was going on and he was out in no time back to his normal self Trust me you will thank me later if you truly love and care for your care or loved one this is the place to go.and they take Medicare I'm not sure about Medicaid but it's Wirth asking believe me. He'll even the food good. My Dad never had repeat food theirs always 3 options the fried shrimp was his favorite. They have a juice and tea and ice station available to everyone even guests 24/7. I can go in forever I'm telling you love it

I cannot adequately express the profound gratitude I feel for the exceptional care provided by Dolphin Point during my father's challenging transition. The staff has demonstrated genuine concern and offered comfort in the most compassionate manner. It's regrettable that my father couldn't have been admitted sooner; the previous facility, Riverwood, was a complete nightmare compared to this haven. Dolphin Point, with its collaborative care, impeccable cleanliness, and exceptional staff, is truly unparalleled. As a nurse practitioner, I recognize the standards and expectations for individual care, and Dolphin Point has surpassed them all. The administrative team, Director of Nursing, nurses, CNAs, housekeeping, and maintenance staff have been truly remarkable. Despite the heart-wrenching process my father is going through, I find solace knowing he is under the care of an extraordinary team. Thank you, Dolphin Point, for your unwavering commitment. It's clear why you have a waiting list.

This place treated my PawPaw like family. And us too. He was in bad shape when he came here from Shands. Within maybe 2 weeks he is not only back to his old self but healthier then ever in a long time and moving more freely. The place is so clean the staff is very nice and friendly. The food was good and they always have plenty option and can get juice water tea and ice always. They never get bored it looks like a apartment place almost. Warm comfy place to be. My PawPaw Mr. Coleman leaving today and sad to go. Thank you for everything everybody

In "ALL" my years of employment, I have never worked in an establishment w/so many RUDE, unhappy, disrespectful, HOSTILE and I mean HOSTILE, TOXIC individuals! The newest hired DON needs to be reorientated on "effective communication" needless to mention; positive feedback with staff and understanding the importance of displaying professionalism and general manners. She is by far
"not" management material (ask any of the nurse and/or CNA's) that presently or formally worked under her leadership! The Facility Administrator (what a joke) speaks faster than he listens. He needs to fully understand; in order to fully "LISTEN" to someone, he needs to first remain "SILENT" for once. Considering, "Listen and Silent" shares the same letters.

This particular facility has an incompetent, bully in leadership with such poor work ethics! This reason alone happens to be one of the overall factor (s) as to "why" a number of nurses have quit (including ME)! And more are planning to quit VERY soon! This also explains "why" they are currently offering a sign on bonus for prospective nurses to join (as mentioned above this TOXIC, dysfunctional team)! There are some wonderful CNA's and very, very little great staff nurses on staff if I must elaborate on something positive. However, this facility is forced to utilize agency nurses again because not very many permanent staff nurses elects to continue working in an unprofessional, disrespectful, unaccountable, hostile work environment! The facility itself is pretty clean in which it should; being that it is pretty much newly construction building.

Word (s) of advice: be aware prior to seeking EMPLOYMENT there. IJS: -/

I have worked at this facility for about 2 years as a nurse on the floor. Most of the staff is still the same. I truly believe that the management team here is fair. The residents are top priority. This is a great place to work. The facility is well kept. I promise that most of the staff would say this is the best place they have worked. I would say the same.

My mother has been in another facility twice and it was horrible the last time so I decided to give Dolphin Pointe a try based on their rating and I was not disappointed. They took great care of mom. The facility is beautiful, clean and doesn't have any unpleasant odors. If the situation arise again for mom to need to go to rehab, I would not hesitate sending her back there. The staff were all very nice, helpful and cooperative.

I work in the healthcare industry. Both of my parents, my aunt, my uncle, and several family friends parents have been at the facility for short term rehab. Excellent service, great communication. Staff is efficient and empathetic. Admission process is very smooth. I recommend this facility to all of my friends and family.

Dolphin Pointe, in my opinion, is the preeminent health care facility in Arlington and Jacksonville as a whole. If you need rehabilitation or skilled nursing care then do yourself a favor and visit Dolphin Pointe. They also make great community partners.

My Mom has been a resident at Dolphin Pointe for over 6 months. We have been very pleased with the level of care she has been provided. We have recommended Dolphin Pointe to several families and friends who were seeking similar facilities for their loved ones.

What a great team they have there. Everyone treated us as family. The therapist and CNA’s were top notch. This changed our perspective on skilled nursing! I would definitely recommend this to anyone that is looking for short term rehab!
