We were informed that the service is not covered by insurance and thus paid out of pocket. Then I contacted insurance and was told they cover and the provider is in-network. When I asked them to bill insurance and reimburse paid amount out of pocket, they refused. Their rationale: "our billing doesn't want to deal with it and we prefer to bill customers directly".
