This was a nice facility. The staff were great and I felt like my grandma was safe and taken care of. Bonus it doesn't have that stuffy feeling/smell.

My Grandma was in here and she received great care. The staff was excellent and therapy was able to get her home with home health. We really appreciate all the staff.

My mom has been in the care of this facility for only a few short weeks and not impressed at all thus far. Patients appear to be left for long periods with no assistance when needed as was evident by all the screaming residents. Upon visiting my mom for over an hour they did not answer her call light the entire 60 min + and when someone did come in as I exited they said WHATS THE PROBLEM HERE as if inconvenienced to be called and not realizing I was her daughter. Communication is also horrible… they have been told multiple times that I am my relatives contact person as well as my sister and yet they continue to say they have no record of my existence and that they don’t have a number or permission for me to be given Information each time I call despite them having taken down my contact info over FIVE times maybe more and my mom giving consent right to her social worker in front of me. The nurses station does not answer the phone whatsoever and when they do they have no earthly idea what is going on with this patient or even where she is. We had an emergency situation where I needed to talk to my mother and I tried for 2 days straight to reach her and her room phone was disconnected so I left messages for someone to please assist her with calling me back, as she is very weak and cannot answer the phone anyway and is confused …nobody returned my call or helped me during these entire 48 hours despite continuous messages left around the clock that I needed to speak with her. I am almost 2 hours away and don’t have the luxury of walking in to speak to my mom at any time.when I finally reached a nurse I was told she was “showering “ and to call another time but I asked her to be given a message. I came to find out at that moment this was in fact a lie and she was receiving treatment at the dialysis center who was professional and kind enough to assist me …how do staff members not even know where their patients are, or do they just lie to get people off the phone? Do better.

Would give 1/2 star if I could Worst place for my dad. Shanon was terrible, highly recommend you DO NOT PUT A LOVED HERE they just don't care about the patients at all stay away. My dad fell out of bed twice

I have an uncle that was recently placed in this facility. And the issue that I have is not in reference to his care as I am honestly not that familiar with what’s going on. But I had a conversation with the LPN by the name of Kristina, who decided to report our conversation back to my uncles daughter. I simply implied to her (when inquiring about my uncle’s current condition) that we weren’t communicating. It only caused further confusion in our family. You would think that a medical professional would act such. PROFFESIONAL. I was very disappointed that this type of situation happened. It was reported to the Director of nursing and I’m unsure if any action was taken or not. But I feel that it was very unprofessional and messy for her to relay our conversation back to my family member. It was taken totally out of proportion. So basically, in other words, be careful who you talk to and watch what you say. As for myself, I won’t bother talking to anyone else about my uncle.

We put my mother on law there for rehab last august and within 2 months she went down hill got a bed sore had some of her new clothes taken.needless to say my husband n i were going everyday to see her.we would bring her food to eat and drinks
when we would askfor water they brought water but it haddirt in wwe found lut she had a big bed sore
that required surgercy.she ended up going to hospice npassing away 13/23/2022.ayer.in ocyber i get call to come pick her stuff up.sadley she went to blake and thats when we

After visiting a dear friend on several occaisions I am appalled at this facility. There is very little to no care, neglect, terrible food, and rude staff. My friend had an injury that went unaddrressed for many hours and then carelessly dealt with. She is also being given more drugs than she had when she arrived and they dismiss her questions about them. Uncooked rice, brown water soup, absolute slop! She went without water for over 6 hours! Sad, they can not even look forward to an edible meal! I am filing a complaint with the state of Florida. I suggest you run from this place and investigate fully if you have a loved one there.

As a health care professional, I have been very impressed with the quality of care that my elderly mother has received at Heritage Park. This even more stark compared to an awful experience my mother had at another facility in the same town.

Phys care person threw shoes at 86 yr old woman at therapy and told her to get to her room anyway she could. Elder abuse.

If I could rate it any lower I would. If Deb Brown is still in control of this place do not put your loved one here. This place is an absolute death factory! Not only did they give my father the wrong meds. We found some on the floor. Then they continued to give them after we brought it to their attention. Then days later they still continue to give him the wrong meds. They ended up sending him back to the ER for overdose. This place should be shut down. I am seeking legal advice the only way this place is going to change if someone sues them. Do yourself a favor just call and ask if Deb Brown is still working there. Not only is she a terrible person none of her employees respect her. And I think management is just happy to have a warm body.
