Life Care Center of Orange Park

After 6 weeks of my husband being in LCC of OP they wanted him out of there! After 4 weeks I had to appeal for more Physical Therapy and I won my case after I was told very harshly by the Social Director that I was too late to appeal, but I did call QIO and won my appeal. I should not have had to appeal anything if a caring facility wants to work with the patients to help them get stronger and walk. Not a “Caring” facility. On top of all of this the Executive Director, Darren Tony, the Social Director, Mark Singleton and the Case Manager, Lourdes Santiago-Williams… ALL have rude customer service, not polite to me and very harsh in speaking with me about the concerns with my husband. I would not recommend this facility or any of LCC to anyone. I will be writing a letter to there Corporate Office and calling to speak with the CEO in TN. Not once did I meet or speak with his physician and I asked multiple times for him to call me… no call at all from Dr Chalhoub

My grandma was their for only 2 weeks after she had surgery and she came home and fell not even 12 hours after discharge then they told her that she could not come back because just to come back they needed 4 thousand dollars worst rehab and we took our business somewhere different

Very nice facility. My sister being well taken care of. The physical therapists are wonderful with her. Continue to be the best rehab facility there is. ️

Understaffed for night. Nurses and CNA must be over worked and underpaid because they’re angry at everyone except their favorites, the lucid bubbly gals. If you can have your loved one at home, please do. This place will leave them yelling for help because they have too many patients.

My experience at Life Care Center was most enjoyable. I was treated with kindness and great respect from all the staff and workers. The facility is very clean.

Check in was easy… went straight To patient I was visiting, I did not have any interaction with a nurse, CNA or anyone other than the lady at the front desk

My mother went to this facility after a week in the hospital. We were greatly disappointed in the care provided. She went there for rehab to regain her strength. She went for four nights without having her C-PAP and oxygen, with me asking for it each day and someone checking it off the checklist in the computer each night. Another disappointment was she was left to sit in BM for over an hour, at which time I had to go ask for help again. Rehab was pretty much a joke. On several occasions, if she wasn't able to perform as requested the PT stopped for that day. A Hoyer Lift was used on another occasion to get her in a wheelchair. When PT was finished, and she was taken back to her room she was left to sit in pain in the wheelchair because the lift had not been charged. After waiting 45 minutes for the lift to charge with no success, I asked for something to be done. A nurse went to another wing and got one. This one lost charge while my mother was dangling in the air over the bed. There is so much more disappointment. Please understand that there were some wonderful people on staff and there were some real lemons. An RN named Erin was wonderful, as well as two therapists, Jim and Charlene, and Sheila a CNA. This place advertises being voted number one by Medicare - if only Medicare knew the reality.

The place is beautiful, spacious and smells nice. But, the care my sister received was less than I expected. I never met her physician (M. Edwards) assigned to her. His assistants came to check on her. I never saw her P. A.listen to her heart, even though she had cardiac problems. He was not that concerned about her care and came mostly early in the am when family was not present. My sister had slurred speech and could not give accurate info on her care. The nurses, CNA’s seemed to be nice, but they had less staff at night. It’s bad that she had to sleep in a cold room at night, when no one responded to the call button to change the room temp. She had diabetic sores and they got worse and later developed bed sores from not being turned often. I did not meet the DON until there was a complaint regarding her care. The food seemed to be good, but suggestions I made regarding her diet were not followed. My sister died in this place 10/31/22, and I have yet to be reimbursed for her shirt that turned up missing. I wish that the care at this n.h. Was true to what they claim to be to the public. I pray that they will provide better care to their residents.

Hate this place so much my father has MS which is very bad and my father was there they wouldn’t give him medication he can’t walk and he had to use the bathroom no one cared he has an infection and it went to the blood I would never put anybody in this place.

Excellent food curtious staff very clean would highly recommend. My aunt was in here we visited her and had lunch with her. Her name is Nancy Waldrup.

Great facility! My dad was at the facility for rehabilitation after he fell and broke his hip. All the staff are so helpful and pleasant. The facility is clean and smells nice. All nurses are compassionate. The food services could be improved a bit. But I understand it is difficult with all the specific diets for all the patients. Thanks for taking care of my dad.

Great visit with an old friend. Staff very welcoming and helpful with a smile. Very clean living spaces and general areas. I enjoyed my visit.

From personal experience, I’d highly recommend anyone to this organization/facility. The level of concern and compassion that is demonstrated among all the staff goes above and beyond. Caring for people during their most vulnerable moment is no easy task, but these folks take caregiving very personal and always put their best foot forward. I’m able to leave my loved one under their care without any concern.

My Mom stayed and some people were excellent but the place in general with the caseworkers was a disaster. The caseworkers are so uncaring, you can tell they are just getting thru the day. St Vincent’s Hospital was even worse.

A very professional staff, nurses, CNA's and management always there to help you and take care of any problem you may have. Many scheduled activities for all the patients to enjoy and have a beautiful day. I particularly like their attitude in taking suggestions to improve the life of the people that are housed in their facility

My husband was a patient in Life Care Center and received outstanding care. From the therapy, to the food, to the nursing care, it was all excellent! The Director of Nursing was phenomenal! She answered our questions and addressed my husband’s needs. 5 Stars to everyone at the Orange Park Campus!

This facility has outstanding service all around from their admissions/marketing team to the activities dept, nursing dept, therapy dept to your discharge with social services. You can count on them to make you feel welcome while you are there to rehab and return home! Consider this 5 star rehab for your next loved ones nursing care needs.
