I'm giving one star at this time, having been inside only once.for whoever decided the interior color and patterns to be a welcoming design. Outdated and depressing. If the decision was made to hide filth, it hasn't served this purpose either. ️
The help seems nice. The air conditioning in room making loud annoying noise. If patient has different experience, will update review accordingly.

How are we supposed to speak with anyone, when no one answers the phone? I have left over 10 messages in the last month and not one returned call. I can’t believe no one has voicemail in that place. I want an appointment with a director and can’t get through. This is not my main grievance with this facility, that’s why I’m trying to speak with someone. You really don’t want me to air my main issue, because it’s unforgivable. I want a different phone number, to reach a director. I’d also like your name “Owner of facility”, as that’s how you sign all of your replies. Thank you!

Inhumane

My partner is in there now with AWFUL care, just out of knee replacement surgery and has had NO PAIN MEDS! They are saying the dr from hospital didnt sign the script!? !? ! We are 24 hr in at this place and still no meds. My partner was left for 40 with her light on in need of urgent care! GET HER OUT OF THERE!

Update: I did call. Roseanna was rude. She said Kim could go home. She didn't need to stay there. I said that's not very professional she just had knee surgery she needs therapy she needs meds. How about someone from the care team reach out to the patient and or me. The patient also shows signs of bed bug bites on her back. Please get her out of there please get her transfer to Brooke's rehab.

My stay here was amazing, everyone cared for the residents and myself with so much compassion and showed love as if we were truly family. I would recommend this facility to everyone.

I visited a friend at the facility; the staff was nice and friendly. The building is a little dated but clean.

Some staff great but got the feeling that most staff were sitting on chair on their phone while call buttons buzzed.

What can I say. AMAZE CARE from when she arrived until now. The staff is on point with her care, the rehab department is all about set goals and reach them and the nurse and doc care about keep you healthy. Easy access to anyone at any time of day or night. Safety was also a huge plus to my mom and family as well.

I really do agree with everything that has been said about this place it just not the place you want to send your love ones at all and I’m a full time employee that works here right now and it’s very sad of the conditions of how this place is I hate how even something coming into work due to all the bugs they have here I have to wear my hoodie on all night just so I want get bitten by all the nets that they got in here it’s just not an up to dated facility however the staff that I have worked with so far have been great…the only person that’s has been nasty to me without even knowing me or knowing that I’m working here as a PCA to get CNA license is mrs Hankerson!

This place does not deserve a star, PERIOD. I give it a less than one star rating. I been here here almost two weeks and I'm witnessing at first hand how bad this place is. Beds hardly ever get changed. Bathrooms never cleaned. Meds don't get out at times (a danger). And don't let me get started on the food. Its a wonder why people die in places like this. The nursing staff is the pits here. Some are good BUT, too many bad ones are kept employed. One good thing about this place. Living to tell my story (hopefully) I'm not out yet. May God help us all.

I stayed there several months. The food is rarely worth eating. Rooms have bugs/roaches of all kinds. Housekeeping is terrible. Employee restrooms are clean, however resident restrooms were never cleaned well in the 2 rooms I was in. CNAs & nurses show very little to no respect to residents. I received disrespectful snide comments from both nurses and CNA‘s and even had a CNA curse at me for politely asking for some additional margarine for my oatmeal.

This is not a place you want to send your loved one. I saw mini mini bedridden patients that the aids waited till after their Break before they would change incredibly wet briefs or even worse.

I heard back from patients who transferred to the Orange Park location say it was far better… I personally can’t speak for other than I saw.
