Our Son was there for a couple of months and the care was ok! We had a very good experience with Zy and she was always helpful

I must say the staff is very pleasant and understanding. Helpful with transitioning in and out the facility. Therapist really cared about stated goals and during the time he was there, they worked towards them. The food could definitely be better and if you want your loved one bathed, it’s best you’re there to help with the upkeep of your loved one. Zyiara Davis was our social service director and she went over and beyond to handle whatever situation I brought to her and needed guidance on. She deserves a 5.

If I could give negative stars I would.the staff dont tend to the patients.the food is not suitable for sick ppl.the staff dont give patients medication on time. I was able to walk in the facility and no one asked me for I'd or anything. I pray no one sends their loved ones to this facility.they dont transport patients to their appointments on time.

Many many days there are not enough CNAs. At times they have as many as 25 patients or sometimes more. So you can imagine what kind of care we receive. I require total care and I probably get a bed bath once every 2 to 3 weeks. And in eight months that I've been here I have received three showers. My call light goes on answered sometimes for hours and I could go on from there but I'm not

First off, i want to start by saying this place is somewhere you WOULD NOT want your family members to come.this place is not clean, they would have there patients sitting in there own urine for hours and hours. And the workers wouldn’t even know until someone say something about it. Then, they have there patients walking around with no shoes asking for help. And there’s this one worker here that just sits at his desk and do nothing! And as soon as you come in it just gives off a very bad vibe.just overall this is NOT a good place at all!

I was VERY concerned about my sister and her care. Our Case Manager Zy is THE BEST. She has been very integral in facilitating my sisters care as well as working with our vendors to ensure that her infusions continue in a timely manner as well as coordinating with our home nurse for care once my sister leaves.

It is imperative that you have an advocate to assist in the areas where balls have the potential to drop. Zy is that advocate for us. If you ever have to utilize a facility please ask for Zy. She has and is making this unfortunate blip in my sisters health very manageable and has really lifted a load of stress off of my shoulders.

Zyiara Davis and Deborah Reddish have been Fantastic in helping my Dad and Mom with getting everything my Dad has needed. We are very pleased with his health care needs.

No toilet paper for my sister, and she's been sitting in pee since 2: 00P.m. Her nurse gave me her Oxy to give to her which your not aloud to! Me and my mom changed her bed and diaper! Fixed her leg brace and got her ice. We've been up here since 7: 00P. M.waiting on her to get the rest meds

I would give zero stars if I could. My nana was left with urinary sepsis and severe dehydration. Debbie is the only woman that controls her finances and now that my nana has died at the hands of their caregivers, Debbie doesn't even have the decency to contact me regarding my nanas belongings or her end of life expenses. Do not bring your family here. Please.

We admit my dad there he’s 92 years old they didn’t take care of him no food or medicine they totally neglected him i was crying when i saw him pls check this place out before you leave your love once there

I have been to alot of facilities since 2014 when I broke my leg and it healed wrong.but this is the best one I've ever been to and I've been to Palm Gardens In Winter haven which is a wonderful place in 2015 but this place is so amazingly friendly and I want everyone to know that! If you need something they go out of there way to help you and the best part is they make you feel like we are all one big family I love them

My dad has been @ San Jose for 2 years and loves it! They have taken excellent care of him and provided his every need. I’m very grateful for the outstanding staff!

San Jose is a great place for my grandmother. She loves it! Well maintained building and very attentive staff. Thank You San Jose for everything. You Rock!
