My mother spent 45 days in rehab following 11 day hospital stay. The care and the skill of the nurses and the therapists were outstanding. The facility is clean and the atmosphere is friendly. The administration staff is professional and listens. The activities were chosen to include many age groups, as well, a variety of activities. I would recommend
The Gardens to anyone needing care and rehabilitation.

Beware of putting your loved one here. I pulled my mom out after 8 days as they said they were giving her therapy.no they did not. She is now back in the hospital with congestive heart failure. The staff knows who is miserable with their own lives and do not have any compassion for humans who need help. If I could give a 0 star I would.

I would have given the facility one star, but for the few excellent individuals who truly care about people and rehabilitation. A special call out to the wound team. The facility itself is very clean and the floors are cleaned each morning around 8: 00.

The negative review is driven by many factors. First, the aid staff is not properly trained. Some don't know how to dress an individual that is unable to do it himself. When I asked about a particular situation I was told the aides 'can't be trained on everything'. The facility doesn't train on catheter care. Countless times I found the catheter bag on the floor which is the never supposed to happen. A few times from being on the floor, the bag leaked all over, most likely the entire night. Many times the overnight staff would ignore problems so the day shift could deal with them. My father heard such conversations. A particular overnight aid had the ability to help my father into bed one-handed while never putting down her cell phone. My father was also told 'don't ask for too much because you may get nothing at all'. A problem caused directly by the facility was putting my father into a bed with an exposed metal frame than scraped the skin on his calf, requiring additional wound care. The facility should be able to determine the best equipment for patients. The PT/OT team was good however when I asked for a plan of treatment and ultimate goals, I didn't receive an answer. Upon admittance to The Gardens we completed a form about dietary needs. My father needs lactose free milk and more often than not received whole milk.
A note to the facility, it would help if all the employees wore name tags. It would be nice if there was a curfew on TV watching in the room. Someone that keeps the TV on 24/7 is very disturbing to the other occupant. Maybe provide wireless headphones.
When I was looking for a facility I asked the traditional questions about nurse/aid to staff ratio. I wouldn't go by that anymore. Ask how many beds each aid or nurse serves. In The Gardens, each aid is responsible for 28-32 beds, as commented by the aid herself. I would also ask what part of the staff is outsourced. In this instance I believe many of the overnight staff are not employees of this corporation.
Again, there are some very dedicated individuals at The Gardens. I would not put my father or any family member there again. If you do have a loved one at the facility, I recommend pointed questions, on-premises visits (at all times of the day), and a written treatment plan.

One of my family members was recently a resident here for a few weeks. I must say I was quite impressed by some of the staff there by the way they interacted with her and the family, her nurse Sherly, her CNA Sunny and Kellie from Activities Department. Their personalities showed they really enjoy working with patients and know and preform their jobs well. If ever a Rehab center is needed again, I'll diffidently recommend this center.

My great aunt stayed here to heal after surgery for a broken femur. She says the administrator, Joe was amazing and that everyone in every department was above and beyond great. She said they had multiple fun activities for everyone to take part in and that it was an incredible experience. She'd recommend this center to anyone

I am very pleased with the effort the activity department places on keeping their residents' content, happy, and engaged. The department always looks for ways to improve and create a pleasant stay at The Gardens Health and Rehab.

My two and a half months at The Gardens, from August 8,2022 to October 24,2022, after a tramatic auto accident and multiple surgeries, were highlighted by the consistent skill and dedication of the Physical and Occupational Therapy Dept, headed by their fine Director, Cherish.
I will not forget my Physical Therapist, Vivianna. A completely knowledgeable, dedicated professional, Vivianna also brought so much caring and skillful determination to get me back on my feet, I worked all the harder because of her efforts, to walk again. The skill and encouragement of my Occupational Therapists, Marilyn and Brook, brought me the tools and confidence to care for myself when home again. This is an outstanding group of professionals.
Critical to my healing as well, I appreciated the attentiveness and knowledge of the Physician Office, and the Nursing staff and their hard-working Aides. I also observed the competence of the Managers who organize residents' Social Services, the complex Transportation system to outside appointments, the in-house Activities, and all the rest. And I appreciated the daily friendliness and skills of the Housekeeping staff and the hard-working Maintenance men.
The Gardens Director, Joe Hager, has a complex, demanding responsibility. I commend him for hiring, and keeping, some very fine people to work and serve at The Gardens.

82-year-old mother there for 2 weeks never got a shower one time. Did not have colostomy bag had to supply them. Wouldn't take the time to show me how to change them.

If I could give this facility 0 stars I would. My mother in law has been a patient at this facility for over a week. From day one I have had to deal with disrespectful nurses and nursing assistants. When I bring it to the attention of the higher ups they tell me that it's the patient not the staff. I have been in the facility several times and witnessed my loved on being mistreated by staff. If you love your family member that is in need of services find a different rehabilitation facility because your live one will not get the care that they deserve and is warranted.

This place is going to be reported my mother was a patient there they released her she's still not very well they sent her home with another patient's prescribed medication and they did not send my mother's home they would not deliver the medicine to the house and pick up the wrong medicine they laughed about the incident so this is going to be taken care of it's unexcusable

I was here for 2 weeks and recieved excellent care. I had a broken kneecap and their therapy dept did a great job getting me up and walking. Staff is wonderful and the food was very good

I have been working with The Gardens Health and Rehab Center for 3 years and have known Kayla in social services for a few years. She is very professional and goes way above and beyond for her patients and their families. She is a true advocate that works hard to promote safe discharges and to set her patients up for success upon discharge. My clients and families speak highly of The Gardens Health and Rehab Center, as well. They have a top notch therapy department that truly helps prepare patients to safely return home. Nurses and CNA's are kind and caring, too. I would recommend this facility to my own family.

Never send a loved one here. The nurses are mean and nasty. The techs argue over changing and cleaning a patient. ACHA needs to step in and either get this place right or shut them down.

Great place, great nurses, they took great care of my dad, he was there for rehabilitation, unfortunately he passed away there, staff took great care of me and my wife after dad passed, let us stay as long as we wanted, informed us on very important information we needed.
Thank you to all the nurses, aides, and other staff that took care of my dad.

All the aides and nurses were very good to my mom. Therapy was always open and easy to talk to them. I tried to visit 3 days a week gardens was always clean and everyone attentive. This is the place to go for rehab.

My Wife just spent a little over 3 months at The Gardens Health & Rehabilitation Center. 90% of the 2 star rating is due to the CORPORATE policy and procedures the other 10% of the rating is a direct reflection on the 2nd & 3rd shift CNAs and a few other staff members. It doesn't mater what facility a friend or loved one is in you have NO reason to complain UNLESS you are there everyday different times of the day. The Gardens rooms are DRAB and have the feel of an institution. In the beginning the food wasn't fit to be served to even a pig, but by the time she was released and under new management it improved 75%. The old saying to many chiefs and not enough cooks is the case here as there were at the least 4 different staff members who read orders for my wife's endoscopy and colonoscopy and yet couldn't execute the orders correctly. The same appointment she arrived 4 hours BEFORE her actual appointment time (that was so fun for her to have t to sit around all that time). The bottom line is this you as the friend or spouse or parent must be 100% involved and the CHECK & BALANCE, question and re-question everything and everyone. If I hadn't more would have been screwed up. Do not believe any of Greystones Web sight anything will look good when you put icing on it.so would i recommend the gardens NO unless you plan on spending the time to be there hours on end everyday. The Physical therapy team on the other hand are EXCEPTIONAL and is the prime example that everything else should be modeled after.

My parents Hernan and Alicia Gutierrez and their family, a son Hernan a daughter Esther, 5 grand daughters, Brittany, Esther, Michi, Daniela, Marcella, 6 great grandchildren, Venezia, Sebastian, Lorenzo, Luca, Emel, Daciana - we became a part of the Gardens Family 7 years ago on Holy Thursday. Yesterday we recognized with gratitude our extended family, the Gardens. Our father went to Heaven on October 7th 3 years 5 months ago. Our mother went to Heaven on March 13th, 2021,3 weeks tomorrow Holy Saturday. During our journey of physical visits, phone visits, facetime visits, calling in to find out how are parents were - we found a compassionate, professional, understanding communication that helped us thru the transition of our parents' journey. As a Gardens family, we communicated, we agreed, we did not agree, we stood for what was in the best and safest interest of our parents' lives - always keeping in mind that an open dialogue is the best way to continue a healthy relationship. There are staff members at the Gardens that in my heart I know are God sent, Angels, if you will that are there to oversee and ensure the advocacy for human beings that can no longer fend for themselves. Visiting physically I felt, I was coming to my other home, and being welcomed by extended family. It made my leaving less heavy, knowing that my parents had angels taking care of them. As I Esther continue and the rest of the family of Papi Hernan and Mami Alicia continue, to transition their physical presence no longer with us, we are forever grateful for the care, and the love my parents were surrounded by. Thank you, continue to be the Angels God is intended for the Gardens to have in the midst of our loved ones. May this Easter, Resurrection, have each and everyone of you, bathe you in the newness of the freedom of being reborn, and in God's Light. There is a mixture of pros and cons here at The Gardens the fact of the matter does not really fall completely on The Gardens as a whole. But on each individual that works there with no heart or compassion for the patients. My Father has never had to be in a place like this ever and for him to be subject to neglectful care such as when he rings his bell for help and nobody comes to his aide and then I his daughter goes to find out why no one has come to help him and you discover the worker sleeping at her desk it’s not The Gardens fault. But that workers fault for sure so to be quite honest. I’m not completely satisfied at all with some of the Worker’s here.
