I am deeply troubled by your facility's reviews and the apparent lack of accommodation for vulnerable patients. It raises concerns about the measures being taken to ensure their well-being. Perhaps implementing undercover surveillance could provide a clear picture of what truly transpires within your premises. It is imperative that you address and rectify the way patients are being treated, as ignoring these issues may lead to more distressing experiences being shared. The current state of patient care is unacceptable, and it is disheartening to witness the absence of necessary action. I am genuinely surprised that no reports have been made to local news or other media outlets regarding these matters. The priority should always be the welfare of the patients.

My mother came here after a hospital stay in December 2023. She was in the skilled nursing section for rehab in the Cypress Suite 1300 hallway. She received excellent care from everyone here…very caring staff and atmosphere. A friendly environment and a very nice, clean place to visit. Nice living room areas and outdoor space. Also the food is very good too!
If you are looking for a place for your loved one, I highly recommend Village on the Green. My mom was happy here and she left strong.

Every aspect of my father’s rehab was excellent! When it came time for him to be released, the social worker could not have been mor helpful. Everyone we came in contact with went above and beyond to make his stay great.

My father-in-law was in the Rehab/Skilled Nursing facility at Village On The Green from June 2022 to August 2022. We were so excited that a room opened up at Village On The Green for him. It is a beautiful facility and at that time, only one-year old. The Physical Therapists and Occupational Therapists did an amazing job with my father-in-law. My father-in-law could not move in his bed, or sit up when he first arrived and he definitely could not walk. Upon leaving he could roll himself over, scoot himself to the edge of the bed and could get up on his walker and walk without any assistance. He was slow and it took him a little time, but I didn't think he would ever walk again. All of the PT's and the OT's said he was a very hard worker and did what they asked of him. They were awesome!

They have some wonderful nurses and PT's, such as Marva, Manny, and Michael. Chris Atkinson, Director of Rehabilitation was superb! Nessy Altenor, the Community Outreach Coordinator was great!

On the downside, the food was horrible. They definitely needed a new menu and new cooks.

I had a very bad experience with their Director of Social Services, Yakini DeClouet. We had a nice conversation one day and before hanging up, she said, "Btw. I will be calling you next week to have a discharge meeting and go over everything your father-in-law will need for discharge, and he will probably be discharged 2 to 3 weeks after that. " The next week came and went and no phone call. The following week my husband received a phone call about a hospital bed and a wheelchair being delivered to our house that day. Nobody told us anything. My father-in-law was told he was being discharged the next week. I called Yakini and told her and her response was, "o.k" Like yeah, so what. I said, "well, you told me you were going to call me last week and we were going to have a discharge meeting and that he would probably be discharged 2 to 3 weeks after that. You never called me last week and now things are suddenly happening, we haven't had our discharge meeting. Instead of him being discharged in 2 to 3 weeks, we are being told next week. " Yakini set the expectation of what we were to expect, but she did not follow through. In fact, quite the opposite. She then told me she was washing her hands of us and that Chris Atkinson, the Director of Rehabilitation, would be handling my father-in-laws discharge from that point forward. When I spoke with Chris, he said that is the social worker's job. I said, "well, she is saying you will be handling my father-in-laws discharge from this point forward, she is washing her hands of us. " This is in fact the social worker's responsibility and she washed her hands of my father-in-law, because I called her to find out what was going on.beds and wheelchairs are trying to be delivered, we haven't even had his discharge meeting and we are now being told he is going home the next week. Very unprofessional and took zero responsibility. My father-in-law was not released the following week. He ended up being there for another 3 weeks and his bed and wheelchair were delivered the week prior to his return home. It ended up working out, but we never heard from the social worker again. Extremely poor job and disappointment on the part of Yakini DeClouet.

There are several 5 star ratings on this site, but I can tell you, several of them are from the staff, Directors and Administrators that are employed at Village On The Green. Vernon Zeger is the Executive Director at the health center, and Melissa (Missy) Costa is the Administrator at the health center.

The skilled nursing is the worst! I have been a private aide for someone for about 5 weeks now and there has not been one aide who works for the facility who has come in and even checks on the pt. I put on the call bell and they take 30 minutes or more to come and see what you need. The nurse’s lack care and compassion. There has been several situations that involved the Director of Nursing and the lack of care she has shown has been evident. The staff acts as if she’s not a resident of the facility. There was a situation with an aide I asked her to assist me with a change and she came and took the pt to the restroom. I told her the pt couldn’t stand but she insisted. Well that didn’t work. She then tells me she’s going to get a lift called sit to stand. With this machine the pt would have to be able to stand and hold on. The pt that needs assistance can not do either on but OK. It took the aide more than 10 minutes so I went to find her and she tells me she’s busy. I said you left and said you will be right back and now your busy passing lunch tray’s? I said, No ma’am she will not eat like this. The aide tells me again she’s busy and she’ll get to it when she can. Another 45 minutes later she comes back with another aide and the machine. They tried using the machine but was not successful due to the pt not being able to stand or hold on. I suggested they lay her in bed. They did and all was good. I have so many terrible stories about this place. If you place your loved one at this facility please check on them. If they are able to speak listen to them and if they can’t please please check on them consistently. I am a cna and I’m never saying the aids or nurses don’t get busy with another resident but I will say it’s no such thing as your too busy to peep your head in and say, is everything ok does the pt need anything? That’s not ok to go a 12 hr shift and don’t know who her aid is or 5 hrs not knowing who the nurse is until it’s med time. By law any resident supposed to be checked and changed every 2 hrs. This pt has never received this.

I would never send anyone u know here to skilled nursing or assisted living. Look at all recent reviews. What we have experienced is awful. Most outsourced help with nurses and assistants. No one knows what’s going on— no consistency. Employees basically on there phone all day. I don’t know who manages this place but they should have been fired long ago.

I am really impressed with the staff at Village on the Green. They are always available and always attentive to our needs. They truly care. It is a comfortable environment for my family member in this challenging stage of life.

This place has been nothing more than great for my mom. She went after a major surgery and needed a lot of therapy. This team came together and got everything done my mom needed. It was not perfect but they definitely followed up. I give them 10 stars for great follow up and Dr Jean very professional and caring.

My dad came into this facility from a very poor experience at Advent Hospital. The nursing staff was a little slow at first but turned it around. The doctor was very attentive and she definitely earned their place. The Voneberstein family greatly appreciates you all!

Working with the staff here I’ve encountered nothing but respectful, adherent and compassionate people. There is never a perfect facility but they most certainly follow up. Go team village on the green.

Let me start by saying thank you village on the green for being there at a very most challenging time of my family's life. Although everybody's expectations are not realistic some of the families are appreciative to the world-class staff that you retain. I read the reviews about the meals. And I witnessed the challenges the dietary staff has had. It is not easy serving 65 people in a hot delicious meal at the exact same time. Realistically they may be a slight variation in temperature. I saw nursing staff dietary staff all chipping in together to get those meals out and in front of several residents. And always with a smile. I support and love the staff members here at village on the green. I will also like to point out the long term staff members that has been here for years back when I was coming to visit other family members loved ones. These are the ones that should be recognize and a pat on the back. Because they are truly like family to these residents they know many of them by first name. I know my family knew a lot of the staff members by first name and a new me by mine. If no one else tells you veterans of village on the green let me remind you you are appreciated. I know the work is hard. And I see your challenges. We love you we appreciate all of you. Keep up the good work. And thank you from the bottom of my heart and from the Davis family.

This place looks nice on the outside but don't believe everything you see. One of my family members has been flagrantly neglected in this place and tends to be dehydrated every time family members come to visit. The staff don't seem to care about this nor do they seem to be bothered by comfort and compassion. Please do your homework and don't send your loved ones here. It's just horrible

I would leave negative stars if I could. This place is dangerous for the elderly. It appears nice but the care is poor and neglectful to many of their patients. The staff does a terrible job of communicating and the only way you can see if your loved ones are okay, is to show up and make sure they are getting the care they need. There were several times when there was an issue, including an infection that was ignored. Complete incompetence.

This facility is ran by people who are not only desensitized but incompetent. Do not send you loved ones here. My grandmother has been neglected for too long and it’s unfair to her and our entire family. Tbe amount of UTI’s she’s had due to not being CLEANED properly is unacceptable and unheard of. She’s constantly dehydrated and they do nothing to make sure she drinks the proper amount of water she needs to. They do not answer the phones or even try to help assist the elderly in calling us. There’s not one person there that genuinely cares for these people in here. They treat it like just any other job and not like they’re looking after actual human beings. All of the positive reviews are left by people who work there so don’t believe the hype. They’ve been gaslighting my mother who is in macaregiver to my grandmother instead of trying to help and resolve these EXTREMELY bad situations they’ve caused. There no damage control being done. The directer doesn’t or won’t even do a walk through to see how bad it is. If these people were related to the staff I’m pretty sure they’d be taken care of better. MY GRANDMOTHER ISNT YOUR 9-5. She’s a person who is PAYING to have assistance and needs it tremendously and isn’t getting it. It makes me sick these people have no compassion or understanding to this. In this industry there’s NO ROOM for incompetence. Zero. This won’t be the last time you hear from me or my family. Something will be done about this.

My aunt is in the assisted living facility and she has dementia and Parkinson’s. My cousin has been having the worst trouble with her care here. On multiple occasions she’s been there and found my aunt severely dehydrated even though she’s adamant that her caregivers should be making sure that she’s drinking water. My aunt has had multiple utis that have required her to be hospitalized. My cousin has to stay on top of her care and medications because on her bills they have double charged or even charged her for medication she’s not taking. It is heartbreaking to know that this is what we have to look forward to when we need care the most. If this was a daycare for little children people would be livid however it’s so hard to know that when we age people just don’t care about how we are treated anymore.

My mom was in their rehab facility. They were not equipped to deal with dementia. At that time administration was in turmoil. Food service was a nightmare and received very inconsistent aide service. Some very good, others did not want to be there. Physical therapy personnel inconsistent as well. Rooms nice.

Are there negative stars? My experience has been horrific. They do not have enough CNA’s to manage the amount of patients they are providing beds for. My family member is still there, we have had to hire a private nurse to stay with them in the evenings, however they can’t start til tomorrow. You read that right, we had to supplement the rehab with an additional private CNA overnight This morning my family member was left from 5 AM til I got there at 9 AM, in an overflowing soiled depends. The only reason assistance was provided at 9 was because I went and demanded it in person. This is a beautiful multi million dollar top of line facility with terrible management and even worse care.

I worked for this company for ten plus years and I moved to Orlando Nurse Pamela Boshell has been giving me a hard time, she has no compassion. The job only kept her, because she’s bringing money in. I’ve given her a Dr note for my sick baby she said i had 10 call-outs, attendance policy, when I was on maternity leave in between the year, she has been evil in her own manipulative ways; no point of arguing. I quit because of her I did not report her because God knows everything there is more to say, but they’ll understand as they see the employees are quitting, and going back to there old positions. 10 years of waste because of one person.

Frightening experience! My husband came here to recover from surgery. He was here for about 10 days and did great until the last day. Unfortunately that was a Saturday, so the offices were closed or I would have gone for help when the nurse ignored me and left the room every time I said “there is something wrong with my husband! ” He instead pushed us through the discharge process and I realized we had no recourse. We went home, and within 5 hours I was calling 911 and they were rushing him to the hospital. Yes, there was plenty wrong with him, and the nurse’s negligence could have cost my husband his life!
