My name is Mike Ingram. I was the Dietary Manager for a few months. And was terminated after Shady things were brought to the administration. They will respond to this stating I was termed for a bad attitude etc. Which I may have. Since they are refusing to give me my entire last paycheck I felt the need to put this out there. They have have no staff in the kitchen who knows how to properly give textured diets, accordingly to the guidelines. They have cooks there who abuse special needs people by not allowing them to use the restroom and urinated on themselves. They do drugs in the kitchen. I have the text messages from team members sayin they felt uncomfortable with the drug use to admin and was fired. I have been threatened with violence by two team members and a resident and nothing was done. Again I have the text. I had worked 1 month straight without a day off. Fortunately I have lawyers looking into the matter for creating a toxic work environment and harassment. By the way if you have family members needing a facility avoid this place. They are severly understaffed. If you have a resident who needs special diets they will not receive them as we are discouraged to use therapeutic diets. There is no special diets here such as diabetic or heart health. Keep family away. If you have any questions call me I will be glad to share with state about the roach infestation as well. Crown care has amazing many team and not afraid to pitch in and help out when needed and the nursing staff is awesome always helpful and caring, nurturing and they work together as a team!

They have the most active activities department I have ever experienced. Their staff are all very kind as well.

I have been at crown care center for almost a year, I absolutely love working there l love the way I can always go to my DON, ADON, ADMINISTRATOR, AND VOICE MY CONCERNS, AND THEY LISTEN, and most importantly they love our residents

Have been with crown for a year now. I can for sure say the mangement team truly cares for the employees, and residents. The residents are so full of character, and have stories for days. It is truly a great place to work for

Fantastic place to leave your elderly loved ones, whether its for rehabilitation, or long term care. The staff here is loving and caring they genuinely love their residents and the quality of care is tremendous. Administration is helpful with insurance and helping family transition their loved one to rehab or long term care.

I, Ron have been the Executive Director for Crown Rehab and Healthcare for a year. The residents are so much and full of love. The management team and staff here at Crown is amazing. The staff here believes in treating our residents with dignity and respect. We go beyond to make sure we are meetings our resident's needs.
What makes Crown different from other skilled nursing facility:

This Facility has been one of the best places to work at.it’s improved so much in the last year with the quality of care.the residents are involved in plenty of wonderful activities and the staff are so involved with every single person.i love it here, and i hope you and your families do too.

From the time I was faced with making a decision of where to place my father after a 2 month hospital stay I second guessed my decision but kept giving Crown the benefit of doubt. I even met with Michael, Social Worker, Rochelle, Assistant Director of Nursing and another individual and voiced my concerns, which ranged from my father losing a substantial amount of weight over just a few weeks to his pain management, to cleanliness of his room (bedding, etc). I explained to them as his advocate for making sure he was cared for it was important to me that I was included and made aware of his care schedule including awareness of medications. They assured me in that meeting that taking care of my father and making sure he was clean and comfortable was their top priority. They assured me that they would monitor his weight weekly. This did not happen. We continued to see him deteriorate and relied on the experts (doctors and nurses) to advise or suggest hospice. That did not happen. At the time of admission, paperwork (Trust) was provided that clearly indicated he did not want to be resuscitated, but instead that took measures to bring him back until they called me on the morning of 11/28/22 and I told them to stop. I also just learned from reviewing his medicare claims that a new drug was introduced 11/26/22, Megestrol. Reviewing this drug brings serious concerns for it's use for my father - stroke, blood clots. Why was I not contacted and advised of this drug? My father entered Crown on 10/21/22 and he passed away on the morning of 11/28/22. It's been 5 weeks since his death and I'm still waiting for a refund due from prepaying November.
