I love the people here. Very helpful people got me right on my feet made me feel like I was family here and very loving and caring staff.

I found park ave extended care to be a very professionally ran facility in long beach. The nurses have a excellent bed side manner in caring for the patient and the medical staff is very attentive to the medical needs to all the patients. I highly recommend them.

It was very difficult to have my dad there after his fall, he is 92 but thanks to all the people that works in Park Ave rehab everything went smooth, starting from the person that welcomes u in the lobby, to the great social worker, nurses, coordinators, nutritionist, therapist, aids. Is a great team working together to make ur stay pleasant. Thank you for all ur help, caring and understanding.

Staff is very nice and helpful. Always on time for medications. Recreation always does nice thing to keep us entertained. They got me back on track and helped me better myself. Will recommend.

Worst run facility around. Health care is poor and understaffed. You can tell when staff cares but they didn’t, they were only in it for the paycheck. No accountability by supervisor whatsoever. I had major muscle atrophy and couldn’t go to restroom without accompaniment. I would either be left alone in restroom for long periods of time or left with a dirty bedpan for hours. Wrong medication would’ve been given if I wasn’t on top of the situation. Excuse by nurse was that she had pulled an all-nighter and was sleep deprived. They lacked important medication even though it was called in with prior notice. Medication that needed to be given was not being given at the correct timely fashion. PT neglected to put me into computer for scheduling with no follow up by supervisor to assure my session, which was the main reason why I was there. My first scheduled appointment would’ve been five days after arrival due to incompetence. Good luck phoning to get through, my wife called numerous times which no one picked up. I checked myself out after the third day. They thanked me by giving me a bologna sandwich for my last dinner. ‍️ only bright side were the aids who were pleasant and were quick to offer me hot tea and iced water. No one in a higher position ever called me to check on my experience.

I would give a zero star if I could. Nurses aides and nurses all complain about being understaffed and you have patients waiting for hours for bed changes, diaper changes, water, medications, bathing. Then you have doctors coming in all hours of the day and night with no notice while you're sleeping and wake you up and say let me see whatever the injury is The doctors then just say "ok" and walk out. They don't explain anything of what their findings are or what the next procedure is. Then if you ask the nursing staff has anything changed with my medical condition? The nurses say the doctor will talk to you next time he comes in. Then the doctor gives no information again and the patient is left in the blind. They just leave patients in limbo. Then you your have the administrators or directors of nursing that just go along with the Staff whatever the cna, nurse, or other staff tell them without any concern to the patients health and safety. If you have any loved ones in this facility please remove them if you love them. This place is humiliating for people that can't help themself and people left in fesces for hours. If you concerned for your love ones and and want them to have the best of care, Park Ave Extended Care or the other facilities connected to them through my research, is not the place for them. I have found 3 under the same owner. This is all new to me, but if I can find 3, then you can probably find 4 or 5. What I plan on gaining out of this is to have your loved ones to spend their final days in comfort or have your loved ones healed and returned home back to their loved ones with no nightmares of Park Avenue. Why I say this is because at this time I feel the older I get it's a possibility that I may have to return back there or another facility like this. Let's stop this abuse so that our loved ones can be taken care of properly. Remember we may all have to be in the predicament one day in the future. So let's all reach out to each other and change the neglect and abuse. This is all new to me. I'm not educated person but I know right from wrong. Please think about what I've said to you here and as a team we can stop this.

I was a patient here for almost 2 months. CNA Jasmine saw me at my worst and took care of me. We became great friends. Keila and Regis were great as well! Therapists; Farrah, Stacy, Colleen, Boris and Emmanuel were great and got me back on my feet! SW Victoria was very helpful and helped me in getting back home. My friend is a permanent resident and is very well taken care of. I would recommend this facility. Great rehab!

Mr. Cano, we experienced the exact situation with my brother in law. He came after back surgery. I understand the challenges with COVID, but when you see your loved one deteriorating before your eyes, it is hard. He was drugged, suddenly not able to walk, incoherent, malnourished and frightened. They refused to release him to family. Long story short, he is home, eating, walking, coherent, just after one week. This place, the staff, including the social worker, are a nightmare.

Yall really want to keep deleting my reviews but the public and your management needs to know what you did to my family. I will keep posting so everyone can see this, so keep at it. Last January, my father had back surgery done and we choose and trusted this rehab center for him to recover. The pandemic hit everyone and I wanted my dad discharged immediately. They would not let my dad out of the rehab center until they juiced us out of the insurance money and his set release date (which was in May). This nursing home turned into a prison for my dad. I understand the pandemic took us all by surprise but the amount of neglect and treatment my father underwent is unacceptable. I RESPECT all frontline workers, thank you for everything you CONTINUE to do for us but lines were crossed. My dad did not almost die from covid here but the negligence of the staff on the 7th floor that were "watching" my dad. When my father was finally discharged, he weighed 96 lbs (originally 152), heavily malnourished, emotionally disturbed, doped on diazapram (which none of his general physicians or my immediate family authorized), and on the brink of death. Words cannot describe the anger, disappointment, and hurt this has put my family through. The worst part about it, there were NO apologies from this facility when I finally got to pick up my dad. Stay away from this place and look at another rehab center.

Facility is excellent. Aides and nurses are very nice and helpful. They helped me get better to come home to my family.

This facility is very clean, the Aides are their best asset. When you are very needy and your body is not able to move, all the ladies are so dedicated to help you with a genuine heart to make you feel comfortable and assist you very kindness. The nurses are also so helpful. I do recommend this facility very much. I will come back to this facility if I needed their services again. Thank you, Park Ave.

I highly recommend park ave to everyone who needs a lot of care. I was in need of extensive care when I first arrived at the facility. Today I get to walk out of here and I’m so thankful to all the staff Who helped me achieved all my goals. I especially want to thank the nurses that took care of me like Samantha and Sandra they were very amazing. My social worker Cynthia who helped me set up everything for me to go home safety and Alana who has been with me from the beginning. I am so great full to everyone at Park Avenue.

So sad and unfortunately our Friend is there and no body cares about him. Every 3 or 4 hours the nurses walk in the room to see what’s going on. And He has been calling them 3 or 4 hours ago because He needs to use the bathroom. The girls are very rude and cero polite.

The place should be shut down. I came to the second floor and it was haunted. There is a cafeteria kitchen and it was laden with germs, it was unclean creepy and you could tell something wasn't right; They push bodies through the cafeteria to get downstairs where they are picked up by the funeral people. They leave food spilling over in styrofoam trays in the night in a room meant for lunch that is dead and stinking.
