Wrote false information on my health reports on crucial topics due to rushing to fill out my forms for school/work. After my school called to show their concerns on my false health conditions that prevented me from participating in some school events, they filled the forms out again correctly but then lost them. I had to change my doctor as soon as possible to a professional and more conscious doctor who cares about his/her patients. The receptionist lady not once apologized and made me drive each time to give the forms because they don’t have the technology to receive emails in 2022. I would give this place 0 stars if possible. Never would recommend.
