I am a school nurse and sent a letter home to a parent that their student was due for a new dental. The office secretary, Shira Bunick, scribbled a small paragraph on the letter sent home of what treatment the child received and then she or the doctor signed on the line where the SENDER of the letter, me, usually signs. I've never had an office do something like this.
I had to call to request a formal letter or form from the office regarding the student's treatment and was told that it couldn't simply be faxed over but needed to get picked up by the parent. During COVID, if a release form can be provided, I would think that doctor's offices would prefer less in-person contact if possible.

Phone custumer service sucks! Minimum an hour waiting time! Just don't bother look for other primary care you'll save quite a bit of time

Terrible place, I wish I could give no stars. You can never see your primary physician. And they hire a lot providers with no experience and it shows. They don’t provide the care that you need. And you end up making multiple trips for the same issue. I’m definitely leaving this facility because I need a primary doctor to help me maintain my health.

The Shoe Repair store will get you a better diagnosis.
If you care for your health for your kids health, avoid going there. Just a shame
