Sapphire Center is very clean and the staff is wonderful to work with. Our Uncle has been doing well there for 2 years. We were a little worried because of the pandemic, but the staff and administration did a good job containing it.

I can’t say much about other peoples experiences as my ultimate goal was to get out of there but get out the right way. I had broken my hip and had to stay there for 6 months of my life. Now with that said in no way was this easy for me, However I had a very positive stay. The Nursing staff was good and attentive but I had the luck of the draw really and me being able to be verbal helped. The director Ben, Dr. Barbara and Dr Ramsan helped me if ever needed anything. The team that helped me the most was the physical therapy. Dianne James pushed me every day to start walking everyday and pushed me to do all my exercises with a “Hell yeah! ” attitude. Rhoda Bulatao helped me learn how to transfer in and out of the wheelchair and to do everyday things with my current obstacles. Dorothy the director of physical therapy looked after me like a big sister and gave me pep talks if I was ever going through something all while running a great team. I’m entirely grateful for the entire staff looking out for me and helping me get back on my feet. I will miss them. If you were in my situation and needed a place to help you get back to some kind of normal this is the place to go. Hell Yeah!

My mom.has been here several years with Alzheimer's. The staff is always so helpful. They keep me updated. She's been in other nursing homes that were really bad. Really glad I chose sapphire center

So far this place has been treating my father better than expected. I was a little worried after reading reviews, but the communication is very good between the nurse and family. They also accommodate any concerns. The place is clean and you have to get rapid covid testing each time you visit.

Please save your elderly ones and do not leave them here my friends mother was discharged with a foot infection this is inhumane and absolutely ridiculous how you left this poor women you should be ashamed and embarrassed to even call this place a nursing home.

Back in July I got into a terrible accident and I didn’t understand how bad it was when I got to sapphire I was scared only because I was the youngest around and I thought I was going to be all alone there they had also told me I was going to be there for a while. I gave them such a hard time my first night but still one of my first aids cici talked to me and told me they were going to take real good care of me. When I first met my therapist they instantly got me to laugh especially Karen and rhoda and therapy became my favorite part i literally felt like they were family everyone from security Rita and Erica to my nurses and Aids joy and Carmen to the kitchen staff the cleaning staff they would come to clean and greet me and ask me how I was doing always making sure I was good feeling good Karen kept my spirits up all the time and made sure I put my part even when I felt like I couldn’t she would reassure me that I will be back to normal soon but I have to want it also she use to put me to work lol I am so glad I was taken there because they overlooked how stubborn I was at times and my attitude they just wanted to make me better and I’m so thankful for the expierence of going through one of my hardest times around great people.

Horrible place; the nursing are abusive, don't respect patient's rights; they kept my husband with a hole on his throat for months to charge him for a trach that he didn't have anymore for months ;he's got multiple recurrent pneumonia from that. Don't send your loved ones in there! It is not a good place! They are not trustable with competency!

When I first came to sapphire I was pretty depressed and sad because I had just broke my femur from a car accident so I was pretty convinced that I wouldn’t be able to walk again but soon as I came in I was welcomed by friendly faces and smiles who reassured me that things would get better. I couldn’t move my right leg or bend it but thanks to the help of physical therapy I was able to, I have to give a lot of credit to Karen who was my physical therapist she worked with me everyday gave me emotional support which kept me going were days where I didn’t want to keep doing it but she has a very good sense of humor and knows what she is doing little by little day by day I was able to move my leg and bend my knee inch by inch to point where I no longer needed help with daily activities I really want to thank her for everything she did to help me not only her but the whole staff of Therapy was very helpful have to give a shout out to Liz, Troy, Saffi, Jen, Garou who helped me regain my upper body strength. A big shout out to all the nurses who took care of me while I couldn’t do my daily activities I really thank you all for the work that you do my stay here was very comfortable I would recommend this place to anyone, As long you do your part to get better there’s nothing you can’t do with the help of Sapphires staff Thank you for everything guys

I was a patient here and as soon as I arrived here the director told me that they were gonna take good care of me, that’s when I knew I was in the right place.

My experience here was amazing; all the staff (my Physical therapist Karen, occupational therapist Rhoda, Troy and nurses) were super super caring! When I arrived to this facility I couldn’t walk! But my PT Karen assured me I would leave walking! And YESS I left walking! I had a terrible car accident and lost hope (while in the hospital) of ever walking again but with the help of my PT Karen and OT Rhoda and Troy I was able to regain my life back!

Thank you all so much! You guys will always hold a special place in my heart! Thank you for being part of my recovery!

Does not contact the patient's family regarding care, and when they will be released. My phone calls would either go unanswered or they would pass me from one person to the other, still without an answer.

Physical therapists are great and food is good but staff on at least one floor are condescending and one nurse was outright abusive. I was here July 2020 after waking up from a coma with no feeling in or use of my legs. Thankfully I had to be transferred to the hospital then went to a different facility before finally coming home.

This review is solely for the visitor experience. The staff here needs to realize there is an emotional component to healing therefore a patient should have unlimited time with loved ones. I came here with my mom to visit my aunt, we are both vaccinated and wearing shield, gown and mask and also had rapid testing onsite. However staff insists we can only go in one at a time and only for 30 mins. Is this a jail? The whole of Nyc will be open in 2 days and they are being this ridiculous. While I’m all for COVID precautions, logic should still prevail.

This little note cannot express in words how much we appreciate all the staff at the Sapphire Nursing and Rehab facility. They are truly an inspirational team of professionals. We cannot thank them enough for the encouragement and excellent care they gave my father. He received the best care imaginable and it went far and above the medical care he needed. A special thank you goes to Delia, Nurse Hoo, Dorothy, Karen, & Hemali. Every single one of them have made a lasting impression and we are forever grateful to have met all of them. Thank you from the bottom our our heart.

My dad was there for short time. Place is clean. The decorations for the Holiday is Amazing. I can only vouch for the staff I met during the evening shift. Nurse Stacey, Mrs. Aquino, CNA Blanch. This place can do better with some of the staff. I saw some rude CNAs. This Rehab Center has a various schedule for the seniors to get distracted. But overall I really love the place. You do see some seniors without visitors which is really sad.
