The activities staff and PT staff are super here. Unfortunately, there isn't a full-time social worker (to be honest, with the number of beds here, they probably need two) to help guide us should we need to transition out. I got a message today, very vague, no last name from the caller, that my parent had swelling in her legs but no one answered the phone. I know that calling back to speak to anyone in nursing about her condition will be a wild goose chase. This needs to be rectified with someone in charge to communicate effectively with PPA/amily.

I really don't want to give this place any stars it's horrible they leave their patients lying in feces for hours! Staff sitting around looking lost or on their cell phones not helpful at all. The room conditions are so unsanitary, dirty un-mopped floors, unswept, they will not feed your loved one properly or give medication properly! NEVER GO HERE RUN RUN RUN!

If you love your family member, Clovernook nursing home is NOT the place for your loved one to be. My mother almost died as a result of the negligence of this facility. A visitor stopped in to visit mom and found her lying in a pool of blood and feces. It was later determined that during my mom's onsite dialysis appt.that morning, that mom was lethargic. A lethargic patient (mom) was sent back to the room and no one checked on her at any point during the day. The visitor arrived at 2 pm. The visitor had to alert the staff to the situation and even after 20 minutes, none of them went to the room to see about my mom. The visitor continued to advocate for my mom until an aide finally came to the room. At this point my mother had lost over half of her bodies blood content and was having a cardiac arrest event as a result of not receiving enough blood to her heart. In other words, mom had a heart attack. Mom was rushed to the ER. The ER staff was very upset regarding the condition that my endured as a result of the Clovernook staffs negligence. My mom had to have several emergency blood transfusions. My mom is now suffering from PTSD as a result of the Clovernook experience. When I questioned the situation during a meeting with the Social worker and the nurse, of course everyone wanted to pass the buck. I had called the site administrator Keith/Kevin.whichever is his name three times, and till this day, he has NOT returned any of my phone calls. There is a nurse there name Katie. Katie should not be in the health care field. When I spoke to the charge nurse about Katie, I was advised that they are aware of Katies, non caring, non compassionate, and overall lackadaisical attitude and that Katie had been moved off of my mom's floor as a result of my reporting her. I questioned this whereas I don't just advocate for my mother, I'm advocating for all residents at Clovernook. During the 2 months of my mom's stay, I had to provide very basic things such as Kleenex, Depends, Allergy Meds, Cloraseptic Throat Spray because each time my mom would request these items, she'd never receive. Food arrived cold, and sometimes appeared to be not cooked completely. Please DO NOT TAKE YOUR FAMILY MEMBER TO THIS PLACE. I have reported them to Ohio Jobs and Family Services, I have also made a complaint to the dept.of elder abuse and the council of aging elderly.

Personally have experienced only good things here. Good people, help you if you need to figure something out, pretty clean, and very interactive with many activities.

Jay has let Shayla and I bring our pups on Sunday to visit the residents who enjoy dogs. Have been having a great time and the residents are always in a great mood. Thanks for letting us come!

Horrible, I was a very distress full employee working in that kitchen. My rating was very clear, & will not change. I will not be reaching out to you, if you would like to reach out to me you are more than happy to do so.

After a long journey of testing and hopeful treatments for an illness that had taken a grip on my brother in laws life, it was recommended that he be moved into a facility that could provide him hospice care. His life expectancy was about six months. This was a shock to us and an understandably even bigger shock to him.

In an effort to move him closer to family, we searched for facilities that could provide him the care he needed in the Cincinnati metro area. After reading reviews and visiting sites we narrowed our list and presented it to Steve. The decision was made that Clovernook Healthcare was going to be the top choice.

In early March of 2022, a spot at Clovernook opened up and Steve was transported there from Columbus, Ohio. On day one at Clovernook, they met Steve “where he was” on his journey and did the same for us as family. It was a rough road up to this point and they did their best to make it a smooth transition. Speaking on behalf of the family, we were thankful because we saw words put into action.

Steve celebrated his 55th birthday in Clovernook that month and they celebrated him. He was even recognized as “Resident of the Month”. These things didn’t take away the difficulty of the tough days that he would face, but they opened the door to let him know he was cared for and they were there for him.

Steve had a heart for people and a love for community. As his time at Clovernook continued over the months. Steve became part of the Clovernook community as he made made strong connections with several of the residents and staff. They not only welcomed him, but they welcomed us as well. Community is important and community made a difference in this journey.

Steve loved his people at Clovernook, he loved the shade of the trees and the beauty of the shelter area on the grounds. All sources of comfort for him (and us) as the journey continued.

Steve passed away during the early morning hours of October 20th, 2022 under the watchful and loving care of the third floor team on duty that night. When we returned to pick up his belongings from his room that day, we found everything gathered and packed with detail and care. An act of immeasurable kindness that made a hard day a bit easier.

Words cannot express how grateful we are for the care, love and dignity Steve was shown during his time as part of the Clovernook community. To all who touched his heart (and ours), THANK YOU!

Keep meeting people where they are, keep making a difference in the lives of others, know that you are appreciated and keep the Clovernook community strong!

I wouldn't recommend this place to nobody ever! The place is so unprofessional, and they don't care about the patients.
