To be transparent and not make this sound like a bitter ex-employee of many years, I went PRN and resigned because I could not handle seeing my patients dying before my eyes which could have been prevented and have spent years trying to get things changed. All I am trying to do is help a family that may be considering this Facility. In my many years this place has been sold multiple times. In my many years working there (even during the COVID pandemic) things have gotten worse and worse. I used to love working there, was proud at how residents were treated and taken care of, but within the last few years, I have seen so many things that are not only unacceptable, but dangerous, inhumane, neglect and in some cases abusive to the residents that are sent there to receive kindness, compassion and care most of which are in their final years of life and many that do not have families to advocate for them.
I am going to explain why I feel this facility is just not a place I would allow anyone I love be placed. I am trying to be honest and have proof all these things happen on a daily basis!
First let's talk patient care or lack there of.
Patients go days without proper cleaning or bathing. When they do get cleaned up, the STNA's are lucky if there are washcloths, towels, shampoo & soap to perform their jobs properly.
Patients have bedsores from laying in the same position for so long, sometimes in their own bodily fluids, in some cases they become infected because of the lack of supplies to care for the sores, causing health issues even sepsis and then death.
Resident that have feeding tubes are not receiving proper nutrition and I have seen some not get feed for multiple shifts, those residents that have catheters, they are not being changed or cleaned properly, to the point that they have mold growing in them. Yes I have witnessed this myself after multiple requests for weeks to the nurses on shift. This again is a health issues and causes preventable issues with just the proper care.
The lack of proper staffing/training of STNA's (who are truly your loved ones first line of defense and daily caregivers that in most cases really do care). Our request for nursing evaluation on our patients when we see changes or red flags are ignored. And if you are lucky enough to find the Director of Nursing to share your concerns it falls on deaf ears because she is to busy on her phone making "Content" for personal gain.
Supplies, that is a joke! If we have any at all it is not enough to take care of everyone. Rough, cheap paper towels to use to clean up patients after going to the bathroom, the lack of lotion, shampoo, disinfectant wipes, cleaning supplies, it is so bad that I have on many occasions bought them with my paycheck to use.
Now let's talk the facility. The lack of clean towels, wash cloths, linen and gowns is just beyond ridiculous. Washers and Dryers that go months on end without repair and laundry stacked from floor to ceiling.
Hot water heaters held together with zip ties, leaking all over the floor and into residents rooms causing not only fall hazards, but a mold
problem and not enough hot water to give proper baths. Non functioning toilets or overflowing toilets that are not being fixed causing not only hazardous waste but fall risks and sewage smell through out the facility.
Food- I just don't even know where to start. There are wonderful cooks in dietary but they can't perform miracles with what they are allowed to order, residents that have special diets or need extra calories are not even being accommodated.
I in my opinion and experience there this facility should be shut down and the parent company never be allowed to run a rehabilitation and nursing facility

My sister just got here and she's happy.they get her out the bed and play bingo with the other residents.the nurses I met today was just so nice to her even when she was being a little mean. Harley thank you so much you rock

It needs to be remodeled and they need to fix that smell. Also, TAKE CARE IF THE RESIDENTS RIGHT BECAUSE THE NURSES DO NOT HELP THE RESIDENTS IN THERE!

This Nursing Home is a clean and patient friendly place. I have been very pleased with the care from this facility for my loved one. Any concerns you have are addressed! There should be more Nursing homes like this!

Worst facility to place your family member. The administration in that building is horrid, they don't know how to treat their employees thus making them all leave which in turn hurts the residents and how the building gets ran. The place is falling apart from the ceilings caving in (which they just patch up and go on) to toilets leaking for months. Laundry gets done all together in the same ancient machines, no hair nets being worn in the kitchen or gloves, food coming out freezing cold, etc. Please don't listen to the good reviews on here as they are all made up because this place is ridiculous. I took this down because I kept getting phone calls from this company about this post but I believe that the public needs to know how this place is. Always understaffed which means residents don't get the proper care they need. Calling state does no good as word gets out that they are there and they get their act together to seem like they do things correctly. The building really needs shut down!

I want to thank the staff an administrators for the care of my dad. They were such a God send in our time of need. Sara thank you for stepping up and getting him out of Miami Valley! Megan the social worker for all the kindness you showed our family, Angel for the calls keeping us updated Amber too. Sorry I can’t remember all the others nurses names. You all are top notch! You truly care about your residents. Thank so much.

I received my fathers death certificate and there were a couple incorrect things on it that I needed corrected. I reached out to the facility and Danielle was super nice and understanding. She helped me try to find the right person to help because it actually isn’t them that signs it when hospice comes in. Basically through her helping me find the right contacts I found that the funeral home fills out anything above the doctors signature and they’d be the ones I need to contact with the information I needed fixed. Thank you!:)

I would give this place 0 stars if it was at all possible. My experience here was absolutely horrendous. The residents do not receive proper care, all they care about is using people to make a quick buck. The overworked nursing staff always seem so incredibly tired of being there. The administration is comprised of some of the most disrespectful people I met in my life. They have the gawl to call their nursing staff “lowly” and “floor workers”. For working in a field that would typically require a lot of patience and compassion.there is none to be found here. I wouldn’t send my worst enemy here, let alone my grandma.

This Nursing Home still recovering from covid, dining room still not open, and still like a lot of places short staffed. Better times ahead

I would not recommend anyone to take they love ones here! My husband had a stroke & needed to be turned every 2 hours that never happened! He got bed sores within the 4 days of him being there smh! Then also my husband had a tracheostomy that which needed to be cleaned out! They never knew when I was coming! One day I walk-in I opened the double doors he was behind I heard him gasping for breaths pushed call light I ended up having to find nurse to come help, when she came in his oxygen was 82 they worked on him to bring it back up to 96. Then later that evening I came my husband pulled his tracheostomy I came & found him with it in his hands, the nurse response was I just came out of there! When we got to hospital they said it had to be out longer than what they said because his hole had started closing up! The only people that were good is Therapy! If you love your family DON’T take them here… Worse experience ever
