New team members on administration staff, new administrator. The therapy dept has been back to taking community outpatients and back to do aquatic therapy, which they have a fantastic pool.

Park Vista is in the process of rebuilding their team. There is new administration & staff that truly care about the residents and are making efforts daily to improve the quality of care.

I think it's improved so much over these last few months the staff is friendly and the care is improved tremendously I'd recommend 100% that a loved one could get quality care there!

Rehabilitation floor was staffed with great nurses and aides. Once transferred to a general nursing floor most of the staff was uncaring and at times verbally abuse. Patients were told they didn’t have time to get them up to the bathroom and to wet and soil themselves and when they had time they would change them. The food was terrible. I am a diabetic and if my blood sugar got low at night there was nothing available to eat. The nurse gave me part of her lunch she had brought. I had to have my family bring in snacks so I had something if I needed it. I had been a patient there about 2 1/2 years earlier and I couldn’t believe how much the quality of care had declined. In spite of everything there were a few good nurses and aides

Beautiful facility with services for everyone. They have an amazing therapy department with state of the art technology and tools.

I wasn’t impressed with how they took care of my brother he deserved better. Understaffed and certain patients received priority over others. Can not recommend this facility.

If I could I would give to this place 0 stars. The Parkvista from years ago doesn't exist anymore. The facility was sold, and for some reason they are trying to make profit from that place. My husband was there for rehab and was a horror festival. He felt twice, nobody caled me. Missed doctors appointments, no cleaning (took me 4 days to wash away the bad smell from there, after he came home). The place is a liability asking to happen. Run for your life from this place.
To be honest, I would not let my dog be there. Still dealing with issues my husband had there from 1 and 1/ 2 years ago.
Short staff, 1 nurse and one aid for 23 patients.why the state doesn't go there?

We chose Park Vista due to the high ratings and past visits. We just removed our family member from long term care due to severe negligence. Do your homework. You can’t reach anyone other than the front desk, voicemails are full and never a call back if a message is left. Our family member lost a ton of weight and ended up with 7 bedsores in under 2 months. Severely under staffed. It’s a shame.

Worked as an CNA a few years back. WORST. MANAGEMENT. EVER. Wost staff ever and nurses have no clue what they're doing. 1 start is giving them way too much credit.

My husband spent 6 weeks here for rehab. He was given great care. The staff was friendly, compassionate and very caring. We would like to thank everyone for taking such good care of him!

This is such a lovely place on the north side of Youngstown. Right by Wick Park my parents love it here and i'm glad they're somewhere safe and comfortable. Whenever I visit the people here are very kind, if you're thinking about this place I suggest you come and visit for yourself!

What was once the best has turned in to a horribly run facility. It appears owner is more interested in turning a profit then patient care and well being. Seasoned loving staff were replaced with low paid employees, food is terrible, and my relatives received horrible care if any at all. Honestly you'd get better quality care at your local animal shelter.

I’m not sure there is a worse place that you could send your loved ones to be. The communication is next to nothing, the care is minimal because they receive minimum pay because everyone that was paid a higher wage was fired. The activities department is the only good thing and outside of a 30 minute FaceTime or visit you don’t hear from them too often. They have the Ohio department of health and they’re constantly investigating them for a variety of things as well as ombudsmen. Luckily we took my grandmother out of there before she failed completely. I’m not sure how some of this administration sleeps at night knowing how many people and how many families are affected by their carelessness.

The staff is great! The facility however is run as cheap as possible! I had stayed for rehab in 2018 and it was great! I highly recommend it. This visit has been totally different, the facility ran out of bathroom wipes 8 days ago, Aides bought their own to use for patients. I had my family bring me some. Its just ridiculous from what I have experienced this visit. If rating the staff it would have been a 5. If things don't change I see what used to be an awesome facility closing.

This facility doesn’t deserve a star rating, but it was mandatory to leave a review. It is extremely hard to watch the geriatric population suffer in this facility! It has been said that a new owner has taken over ownership and has cut staff in half and it truly shows. They work with one nurse on day shift with 33 patients. It’s impossible to assess conditions, give medication, chart correctly, do all the breathing treatments, wound care, blood sugars before every meal, check vital signs 3x per shift, give insulin, and the list goes on, within a 12 hour period. Not only that, god forbid an admission or two. This facility works with one STNA on night shift.for 33 patients. That’s 33 check and changes every two hours, something is inevitably being missed! That increases the skin breakdown for these patients drastically and delays their care majorly. State needs to be in this facility immediately and put regulations in place to help these people. This facility shouldn’t exist at all, these residents deserve better and should all be displaced into a better caring facility. Whether it’s rehab or long term, if you can absolutely avoid placing your loved one in the care of this facility, do so. If you have a loved one here.stay on top of their care, because you cannot entrust in this facility. This is unacceptable and a shame. Start serving our geriatric population better park vista

Park vista is not a good place for the elderly, long term care or assisted living. Terribly under staffed and ran by agency, residents can’t grasp a routine or have continuation of care with a new care taker every day every shift. Rehab clients are brought to the facility under the impression they will stay on a upscale rehab unit only to be place on an over booked long term care unit sharing a room with someone their not compatible with. The food is terrible serving scraps with no snacks. Families can’t call and check on their live ones directly because their is no phones in the room and the units share one contaminated cordless passing it from person to person. Skin break down is at an all time high in this facility due to patients not being checked changed or turned for shifts at a time. I would not place any one I love in this place. The agency staff care more about the patients then the actual workers the Direct of Nursing does not know the patients personally and does not care to learn them. If the resident is not Alert and oriented they will not be consider a candidate for proper care. It seems like a good place to come until u leave your love ones by them selfs. DO NOT PLACE YOUR LOVE ONES IN THIS PLACE!
