I am a 70 year old man. I entered Avamere Beaverton after a stroke lacking the ability to talk, walk, drink, or eat. I was overwhelmed by my reception. I was given a nice room and immediately cared for by a highly trained group of their staff in all areas including medical, voice therapy, throat therapy, and physical therapy. I was assisted in my every need by their staff. Their staff at every level, were very friendly.

I am going home having 90% recovered. Their therapists will be coming to my home to continue my full recovery. At this point I can eat and drink, talk, and walk.

I will be forever grateful for the family of Avamere Rehabilitation of Beaverton.

Worse place for anyone that cares about their family. Push a button to go to the bathroom and you might get help an hour later. Feed your neighbor but hey skip you because there was an issue with your food card. Too many to list.

I transferred my 84 year old mother from another Avamere facility where she had wonderful care and she was in this facility for 13 days after suffering injuries from a fall. While I do believe the staff are doing “the best they can” I don’t feel like the care is adequate. I don’t think this facility is managed very well and is woefully understaffed. As an example just try calling their number and press 2 for the front desk; for me it is actually answered about 20% of the time. When trying to schedule events or coordinate care it is difficult to determine who is responsible. You are generally passed off to someone else or told “I’ll look into that and get back to you”, which no one ever does. Get used to speaking to voicemail and being ignored. My mother will never return to this location.

My mom is here now recovering from a massive stroke. She is a young, vibrant, amazing woman. So for her to suddenly need so much care has definitely been a big shock and incredibly difficult time for her and our family. But thankfully, she came to Avamere Beaverton. We love their staff and care team! And so does my mom. Nurse Tania, Juan, Hailey, Hannah, Sam, Mona, Megan, Debbie, Elizabeth, Dr. Chiruse- those are just a few of the amazing people taking care of her right now. We really appreciate them and my mom has made significant progress under their skilled care. It isn't easy sending a loved one to a SNF during a pandemic, in fact it feels absolutely terrifying. If you're researching SNFs right now, like I was just a little over a month ago, I want to let you know it's going to be ok and Avamere Beaverton is a great place. They are all one floor, with windows in every room. If you want to visit your loved one at their window, you can. We do all the time. It's tough not being able to go inside, but I appreciate how careful their team is and I'm really impressed with their Covid-19 safety screening and testing procedures. When you go to visit, you'll see how nice the facility is. They decorated the lobby really cute for Halloween and even did a fun, Nightmare Before Christmas theme. If your loved one likes bird-watching, this is a great place for them. You'll see bird feeders by almost every window! They have a really cute courtyard too. My mom tells me everyday how sweet the staff are and how much she appreciates them. As her daughter, I'm constantly worried and wondering how she is doing, especially since I can't go inside. I promise you, this team really cares. They appreciate proactive communication and you should never hesitate to reach out and be a part of your loved one's health advocacy. Anytime I was worried about anything, I was always able to chat with someone who helped me and made sure my mom was ok. I'm so glad we chose Avamere Beaverton, they've made a big difference in my mom's recovery and for that we are forever grateful.

I was a patient for 17 days. Everyone was friendly, helpful, and professional. Food was okay. Not much taste, and portions were small. Four occasions I had trouble receiving my meal. Twice I didn't receive it, and twice it was given to someone else. Cleanliness was barely adequate. The bathroom smelled bad, and needed a deep cleaning. Attention in answering the room call button needs to be addressed. Many times I waited over an hour for the call button to be answered. Once it was over 4 hours. A serious problem if I had a life threatening episode. The therapists were outstanding, as were some of the nurses aides. Laundry services were lacking. Often I had clothes misplaced, with some actually being lost. A better way of marking the clothes needs to be done.

First night for my grandmother, she asked me to ask the nurse for her insulin, had to ask them like 5 times to 3 different people and they all told me the same answer that they will check, to pay $1352 for this place to not even bother to figure out what is going on with her, 45min later im asking another nurse who also says she will look into it, smh!

The entire staff is caring, nurturing, friendly. Not only do they work tirelessly but they do it happily. My father spent time in both the Rehab facility and the Skilled Nursing facility departments. Both teams were amazing. Send your loved ones here.

My mother was placed here after we spoke with someone at the hospital she was at. We were given 3 choices and we picked Avamere Beaverton. I am glad we did. She was attended to often, and given plenty to do as far as physical therapy. She was also given cognitive tests. They had very nice things to say about her, and if her direct line wasn't answered, I could call the front desk and they would immediately check to see if she was asleep, in the restroom, etc. I am so sorry that other reviewers have not had a positive experience, but I wanted to share ours that was. Thank you Avamere. Mom is home with us now:)

Not very customer friendly, it seems that no one can answer a phone. I've called multiple times, over a span of 2hrs. This is terrible when you have family members facilitated here and cannot reach them. They need to focus more on patient support and family/patient customer service.

The staff acts like they are being put out of place when asked to assist.nurses say they are busy but when you look down the hall, only see two call lights going off and nurses laughing and joking, while residents are calling for help, As I walk into the room to visit with my father he is almost off his bed and had been waiting for over an hour for his pain meds.again nurses say they are busy but as i walked in there is four at the nurses station.which was upsetting considering phones calls had been made to talk to someone in charge to get my father help.reason we went to visit is because he called and said he wasnt getting the help he needed. I count 6 of them just standing in the hall talking about unrelated work issues.for over a hour I watched two call lights go off and not one aid go in to help. I understand being busy I have worked this field before its a tough job, you have to have the right kind of caring people, this place clearly lacks in that department.

Poor facility, staff does work hard, but they are overworked and under paid.they can keep good people. This place is really horrible in regard to medical care. Don't put your loved ones here, ESPECIALLY long term care.read other posts and see they are canned reviews probably from staff. This place sucks

To start they treat their residents like they don't know anything. Second when I was visiting my mother who has only been there for less than 12 hours they started mocking her and talking trash about her and didn't realize I was listening. The communication skills are very bad we asked for several things and if they don't have it or can't give it to you they don't tell you they just leave you hanging and when you ask again they get mean like you are inconveniencing them. Also I walked by the medication carts a few times and they had medication just sitting out different kinds. Now my mother takes narcotics for pain management I can only imagine how easy it would be for a guest to walk past the cart and just grab any medication there no one was by the carts and I stood by a cart for a good 4 min and no one cares. Didn't come to the cart nothing just left residents medication and information out in the open. So if you want to learn about any resident there just ask to take a tour and I'm sure you can hear about anyone you want from so called professionals. Also I'm contacting an attorney because several HIPPA laws were violated at this facility. Do not under any circumstances send loved ones to this place.

Family and Friends I am posting this because I do not what your parents and your families to go through the same as my family has gone through. It was very stressful situation. At the time Mom’s doctor said she needed to go to rehab I thought it was the best because they would provide care and therapy that she needed but if I would have known then what I know now I would have taken some time off to care for her and also arrange her caregiver to come help us and arrange physical therapy at home.
On the day of admission to Avamere Beaverton, I called the facility to make sure they had someone that spoke Spanish for my mother, who had recently had a total knee replacement. Maria answered the phone stating yes. I then asked her if I needed to go to the facility immediately or if I could go home to get her clothes and check on my teenage boy because I had spend the night with her at the hospital and then go to Avamere. Maria said I was not needed and that I could go home first.
When I got to Avamere at about 5 pm, I found out that she had a roommate that was not very nice and racist. At the moment I walked into my mother's room, the roommate was talking on the phone saying that they roomed her with “a Mexican that's didnt speak English” and “we were in America”. My response was “I understand you” and “you could be nicer”. My mom also informed me that she was uncomfortably hot in the room, which had the door closed and the heat on per the roommate's orders. The roommate also told my mother to turn off her TV and that it was too loud and also that all TV’s in the room turn off at 10 pm. My mom does not speak English but understands it because we speak English at home. She also tried to call me to inform me of what was going on with her at the rehab center but the phone provided in her room didn't work as well as the TV remote, which that was the reason the TV was too loud. They also had not provided an interpreter to help her at all, let alone to call me. At that point they had violated her patient rights.
We owe it to our elderly to report any abuse, neglect, and discrimination. So maybe one day rehab places are safer and better for our elderly.

Friendly and attentive staff. Young and full of energy, keeps the patients motivated for a quick recovery. Nurses are knowledgable and work very hard. Very busy place

My experience has been great! The girls are happy to be here! I think the Peale who has a problem with these great people need to look at yourself! ! I stay here in room 12.these girls work hard for there little pit of money that they do get to

My friends mom went to this place and it was a horrible experience. They didn't provide OT or PT basically she was stuck in bed for 3 days when they should have been walking her. They didn't even give her a shower or wash her clothes until the daughter kept complaining. It really was a disaster don't allow your loved on to go there.

My grandmother was there till they killed her she was susceptible to pneumonia so they opened her windows and left them open during winter and they refused to clean her when she soiled herself they are murderers and thieves 0% Cate 100% big dollar

This place is pretty crappy, try to avoid it at all cost. The help doesn't really know what's going on, nor do they seem to care. No ADA bathrooms in a care facility filled with people in wheelchairs. This place need to be updated, or just shut down!

Place is really run down. Short staffed, takes longer than it should to get help and have them answer call button. I don't blame the staff, they do an excellent job, there just is not enough of them. Especially at meal times. After rehab in the morning, my mother was left to be in bed the rest of the day. If they would take her to the dining room, they would leave her at a table by herself and then she couldn't get back. No socialization of patients. This is my first experience with a parent in rehab, not sure if this is normal or not. If it is, I would rather bring my mother home and care for her myself.
TV's didn't work in several rooms, her rooms were cold. Was told there was no way to fix heat or TV until Monday (this was Saturday so she had to be in a very cold room for two days).
