Evolve Physical Therapy, Sherwood OR

I was very impressed with Melisa and the entire staff at Evolve during my ~2 months of weekly PT visits. Their goal is to not have you come back time and time again (unless it’s needed), and I definitely felt that the exercises were being customized for what I was capable of, not just what fit a particular set of steps. The ability to use the mobile app to keep track of your progress and walk you through your routines while at home was quite handy. Melisa and staff are extremely nice, I felt like I made noticeable progress, and I left understanding what I need to do in order to further my progress without repeated trips to PT; I can’t ask for more than that!

I experienced problems with my neck, shoulder, low back and foot during the last 15-20 years. I tried several Physical therapy providers in the past unfortunately not that successfully.
At some point of time I decided to try Evolve and it was really great decision as Matt Whitaker (co-founder and MPT) was able to help me with some of my health problems in only 2-2.5 months.
Matt asked me all the relevant question, analyzed all my problems and limitations and he was able to detect issues that no one was able to "fix" before, by taking me through the right physical therapy treatment, assigning the right exercises and sending "post treatment" guidelines on "MEDBRIDGE" mobile app.
Unfortunately I will not be able to continue my physical therapy treatment in "Evolve" as I am moving out of state this week, but I would like strongly recommend this place and Matt with his team for the best Physical therapy experience that I ever had.

I had a great experience with Scott. I went to him for a second opinion because the exercises and advice I had gotten from another physical therapist were making me feel worse. Scott's advice and the one exercise he gave me have been so helpful. So much that I haven't needed to go back for a second visit. I highly recommend him.

Matt is extremely knowledgeable when it comes to recovering from injury and returning stronger. I sprained my ankle on a Monday night and he was able to see me Wednesday morning, Evolve PT will do whatever it takes to help you recover from any injury.

Evolve is absolutely fabulous. I've been going to Matt for 6 months to rehab my knee after two ACL surgeries and a recent injury. I've had a lot of PT experience in my life and this has been the best. Matt takes his time to fully understand your goals and challenges you while not pushing too far. He provides great education which I appreciate. I'm stronger than I was and I'm getting stronger every day. Important note - I would consider Matt to be a Health at Every Size (HAES) provider and would recommend him if you're in a larger body, like I am. Never has my size been judged or treatment limited, like I've experienced in the past. He treats you with dignity and respect. Just because you're fat, doesn't mean you deserve to be less mobile or in pain. Matt will help you get the strength and mobility you're looking for no matter what your size, shape, or ability.

I had the best experience here with Matt and all of the staff! I suffered a shoulder injury and was in a lot of pain. They got me in immediately and Matt helped me through my recovery and healing. I highly recommend them!

Fantastic treatment and service. They look out for your best interest and help you get back to doing the things you love. A holistic and unique approach to their treatment. Would highly recommend them to anyone!

My sessions with Matt have been very helpful and educational. He explains what is causing the pain and what we have to do to administer to it. I have felt improvement after every session.
Staff is very friendly and atmosphere is comfortable and professional.

Melisa is incredible! After an ACL surgery, she worked with me at my fitness level rather than the “standard” for my age. As a result, my recovery was so much quicker than if I had gone to another provider. She also addressed as a whole, not just the area of injury. I recommend both her and Evolve for anyone who needs PT!

I am so grateful we found Evolve PT. My 15 yr old daughter tore her ACL playing soccer and did her first six weeks of rehab at another PT clinic and it wasn’t going well. We had often had different clinicians with varying skill levels, and they were working with 2-3 patients at a time. We were feeling really discouraged when we had the good fortune of a friend referring us to Evolve.

As soon as we started, I could tell we were in excellent hands with Matt and his team. All of the staff are licensed PT’s who give their patients one on one care with no double bookings. My daughter has received exceptional treatment and made incredible progress thanks to Matt’s expertise and Evolve’s state of the art equipment. They have really listened to my daughter and continue to create a program catered to a young athlete wanting to return to sports. My daughter has overcome all of her deficits and is on track to making a full recovery. I would highly recommend Evolve PT to everyone.

I have been working with Matt and Evolve for the last 4 years. I was referred to Evolve after I broke my right ankle in a club soccer game. Matts expertise and detailed care helped me recovery super fast. From the balancing platform to the Senaptec goggles, Evolves equipment is unlike any place. So thankful for Josh at Evolve PT. After having my tumor removed I needed PT for my inner ear function. Josh was great, very patient and very knowledgeable. I felt very comfortable and saw improvements right away.

I love the staff here at Evolve. They are so personable and knowledgeable. Josh has been amazing with helping me overcome vertigo issues and navigating getting more help when it became clear that we weren't just dealing with BPPV anymore. After getting diagnosed with vestibular migraines and getting stable on medication, he helped strengthen my ability to handle vestibular input. Then, after having COVID for the second time, my fibromyalgia flaired up and my hips and shoulders gave out. Josh again helped me strengthen my core and got my body back into working order. I would highly recommend Evolve and Josh to anyone dealing with these issues.

I started with Evolve back in April 2022 for PT after shoulder surgery. JP (Josh Pate, PT) was so amazing with my recovery and taught me so much as to what was going on with my shoulder. He is very knowledgeable and explains everything to you so you understand. Now I am back following a total knee replacement in September and of course I am working with JP again. I know I am in good hands. All the staff are very friendly and helpful too.
