Aventura At Creekside, Carbondale PA

As a new team member, I am thrilled to be part of the Aventura Creekside team. The residents are in great hands, the nursing staff has a special bond with our residents, and I can not say enough about the therapy team. They are a dream team to help our residents reach their goals.

I want to thank all the staff for taking such great care of my uncle during his short stay. The administration staff went above and beyond for my family, I couldn't be more appreciative. Thank you all!

True Gem in terms of care and attention, to not only their residents but the overall atmosphere and facilities. The activities and programs put on by the staff truly make it feel like a second home. Couldn’t ask for a better experience for your loved ones.

My uncle is here we went to see 2 days in a row there was blood on his pillow case the 1st day I told staff the very next day still there. He just called me saying he was looked in his room he tried to get a nurse but couldn't reach call bell I called and a guy said sorry I'll take care of it

This facility is very quaint and cozy. The staff are fantastic and treat all residents with respect, always keeping their dignity in mind. The Administrator and his team are so kind and compassionate! They handle any concerns you may have in a timely manner. I couldn’t ask for a better place to care for my loved one. If you’re in the Carbondale area and in need of skilled nursing care, this is your place!

My mother was there for a brief time 3-4 years ago and we couldn't get her out of there fast enough.we were appalled to see an elderly gentleman with his private parts on full display while being transported for a bath.glad to hear it's under new management.

My brother was released from geisinger doing well and sent here. He was there 2 weeks. Nobody noticed the 70lbs of fluid he put on in that time. I took him home and a week later he was dead. Great place!

I have been an employee for 5 years and I love my job! The new ownership and management have made leaps and bounds to make the facility wonderful for the residents and the staff.
The Therapy Department does amazing work with short term rehabilitation.

The staff is very courteous, friendly, and understanding. They went above and beyond to make us comfortable. The situations can be challenging in places like this, and we were handled with kindness and respect. Thank you all.
