Wow, I have never seen a facility go down so fast. When it first opened it was remarkable. Not only gorgeous, but well staffed. We recomonded you to everyone who needed rehab or long term care. All nursing homes are absolutely horrible. We were so glad to finally find one that actually cared. What happened? State needs to know about the elder abuse at your Nursing Home. Also local news needs to visit.

I have had my husband in this place twice. 1st time it was good, 2 nd time not so good. I had to call an ambulance to come and get him to take him to the emergency room. Sugar was off the scale, BP very low. It was that way at the rehab home, they stuck him with some insulin right quick while he was on the girnie. The ambulance driver said what did you just give him, insulin, why? It’s was almost 400, at the ER it was 480 w insulin. Don’t take your love ones there.
My husband is still in the hospital, with very low BP. They probably never checked his BP while he was there. I don’t know how long he will be in hospital. I just pray he makes it. He has lost so much weight, they didn’t tell me at the home he wasn’t eating anything. They put a tray dw and leave. Do not use this place.

I have a friend who is in Advanced Rehabilitation and Nursing. My observation when visiting is, my friend is not receiving care. He is often dirty. On my last visit he had not had a shave or haircut in a long time. From the way he looked and his clothing, he had not had a recent shower and his clothes were filthy. The room and bedding are often not clean. He spends most of his days in a dark dirty room.
On one visit my friend had an IV that was leaking. He pushed his call button and when we left 30 minutes later no one had responded. He has often complained to us that it takes a long time to get help.
With one exception, the personnel we have encountered are not friendly. When visiting we have asked to speak with the nurse on duty but she is never available. Today I called and asked to speak to the patient advocate. The person answering the phone had no idea who to forward my call to. She tried twice to forward me to someone but no one ever answered. She finally took my number so they could call me. I am not hopeful that will happen.
Within the last year my husband has been in rehabilitation care in another facility approximately 100 days so I know what quality care looks like. We were offered Advanced when we chose a facility for my husband. I am so thankful I made a different choice.
I cannot think of a worse place to send a loved one.

This place is horrible. Some of the staff tries to act concern about their patients and others blatantly show you they just don’t care. My grandmother stayed at this facility for almost 9 months and we were fooled because the facility looks great and very clean but we had 3 major issues.
1. My grandma is supposed to be on a low sodium diet so for the first few months they fed descent food but it turned for the worse, everyday they started feeding the patients unhealthy, high sodium worse than a school cafeteria food. Like chilli cheese dogs with chips, corn dogs etc. One day my grandma didn’t get breakfast until 10: 30a because the whole kitchen crew quit and no one was there to make breakfast. Because of the poor diet It caused the second issue. My grandma swelled up like a pumpkin and the nurses would come in her room and tell her how swollen she is this carried on for about 10 days until finally I called the paramedics she was rushed to the emergency room she almost died and had 10 pounds of fluid on her body and heart the staff at the hospital told me the nursing home was negligent and could have prevented the fluid gain If they would have weighed her a couple times a week when we to talk to the social worker about the diet and food they were feeding her the social worker told us obviously she is healthy because she is gaining a lot of weight well we told her she is gaining fluid weight from the unhealthy food. The social worker didn’t seem to care thank God we rushed her to the emergency room and she hasn’t returned to the facility since. Also my grandmother son passed away and we told her social worker she was depressed and she needed counseling they never provided a counselor the social worker told us to contact a church we told the social worker that my grandmother needed a roommate because the original roommate they paired my grandmother with was a kleptomaniac so after they removed the kleptomaniac roommate they left my grandmother by herself they told me they would not put another roommate in the room because the air conditioner unit didn’t work in that room and they were not going to replace it this place is horrible don’t allow your loved ones to live here!

WARNING! Most of reviews are not from actual patients, or family. Their those who work there or friends.

For 5 days they forgot to bring food, if I hadn't literally gone to get food, my mom would have died of starvation. The staff sits around on their phones or sleeps on the chairs. Only once did I see them make haste and that's when they noticed I was taking pictures of them. 90% should be fired! If you don't have enough compassion to feed someone then working with people is NOT something you should do. I have had to ask for clean sheets, I have been the one to give my mom baths. I have to ask for new dressings for her wounds. She still has staples even after 20 days after surgery.the only area that seems to care at all is the PT. But that's only for 30min Monday thru Friday. Shande tries her best, but is only one person and she can't do the work of 100. Fire 50, rehire 35 and increase salary of the 35 with the 15 you didn't rehire. You get what you pay for. Ask for feedback weekly from patients and family. If there's something amiss address the situation. Stop reacting and get in front of this before your closed down!

Visited friend who was there receiving therapy. Beautiful facilities and very friendly personnel. My friend was very positive about the care he was getting and the food they were feeding him. My friend’s (Melvin Strong) wife Confirmed his report and was pleased with his care. Thank you for caring.

Well, initially I have to say that Advanced Rehabilitation and Healthcare has made a great impression on me. And, why not? It is a brand new (open less than a year) facility with state-of-the-art equipment and amenities. The staff there seems to be very professional and friendly. I look forward to more interaction with them.

Seems clean and the nurses seems nice. I work in medical supply and I go in and out of nursing facilities all the time. My biggest concern is the night shift. Most facility's the night shift really don't do much. But this being a new facility im hoping they are better than most of the ones I have been into.

Takes 30 to 45 minutes to get call lite answered. Food is very bad, you have no choice on hot food. They don’t have snacks and sodas. They won’t get you coffee in afternoon. This is a nursing home. Only 30 min pt s day.

Hands down the absolute worst facility ever! From day 1 the care was terrible, people in charge have no clue and will not return calls. If they do not want to deal with patient will stick them in Covid isolation and care gets even worse. Nothing good to say!

Was met at the door and was told to sign in could have had a better system to sign in, but it’s a good security measure….

Diana was so helpful. She has made the entire process of getting my Mom transferred seamless. I can't thank her enough.

My dad called and arranged a pick up time to get my mother with late stage alzheimers, when he arrived they had no knowledge of him picking her up, so when he went into the room to get her, she was still in the pajama top her took her in with No pants, all of her clothes where on the floor. She had a plate of food setting on her shoulder and a dirty diaper. Dried food on her face. And a horrible rash on her back. She was only there for 5 days. We are disgusted that our precious mother was treated so badly. Hospice recommended that he takes her in 5 days a month to give him a break.never again. I hope this reaches other families.

I have a lot to say to these people and would rather not call them out online but please please if you want your family member who is in here because they need help and cannot be dependent on their own, not to die from neglect, do not bring them here. My grandpa just had back surgery and can't even stand up on his own but I literally timed it as they took 35 minutes to come into the room to help. They have been doing this the entire time he's been here. They didn't even come in we went to get them! It's not nice to their patients. I am distraught.

Gorgeous facility. Well kept grounds on the outside, very clean on the inside. Beautiful decor. The staff was very friendly and helpful. I was especially impressed with the PT/OT amenities. The Facility Administrator Sherri was a fountain of information and answered all my questions. In my view, this is a first rate care facility. Highly recommended.
