I have been coming here for almost 2 years! I love the staff and everything about this place. They truly care about their patients. Sara Jaquay, Dr. Dar, and Dr. Crumpacker are lifesavers.

Dr Crumpacker is the absolute best! He is brilliant, easy to talk honestly to, empathetic, teaches his patients and is encouraging. He is rare. I wish there were more doctors like him. He dedicates himself wholeheartedly to help his patients. I have a complex medical case and he has helped me tremendously with managing day to day life and in the midst of recurring pregnancy loss. Hes a busy man so book ahead of time.

This is a two year follow up report on my care. My bi-polar depression continues to be successfully treated using TMS therapy when needed. Dr. Crumpacker has always been available and incredibly helpful in my treatment. The entire staff at SMART Health & Wellness Center have maintained their standard of excellence throughout this time, and continue to treat me with the highest level of dignity and respect as a patient. I am the very much improved man that I am today because of them.

Now, let me I give this message to others who suffer from either clinical depression, or a bi-polar disorder. There is every reason for your to have hope for a better future if you will just find the strength to get the help you need.

You will find no better physician than Dr. Crumpacker, nor a more professional staff than those he has assembled. You are reading these words of praise only because they have been earned. It's time for you to end your silent suffering, and make your appointment to get help. Good luck and God Bless you all.

It's like the right hand isn't talking to the left hand. Here's a couple of examples. The first appointment of the year I filled out new paper work for my elderly parents & myself. Insurance/pharmacy information and such.

After the last visit I scheduled appointments for my mother my self and my spouse.
Just yesterday afternoon I get a voicemail of some rescheduling. After chatting mother's appointment is fine. But my wife's appointment they wanted to push it out until late May. That appointment was for her to see a psychiatrist. She has some serious issues and she can't wait that long.

The original date was this Friday March 13th. So as of close of business yesterday my wife's appointment was in limbo. Last night March 9th I get a text message asking me to confirm an appointment. The appointment is for my wife to see the psychiatrist on the original date March 13th. Which would be this Friday of course I confirmed (yes).

Well this morning after trying to chat and it was to much I had to call in.
Well my spouse will now see the psychiatrist on March 20th one week from the original date. Well at least that's better than waiting 2 months which we can live with.

In my last visit the doctor changed my medication. He asked where I wanted the prescription to be sent. I told him the new mail order service which I won't name. If you recall the first of the year we updated the paperwork. Well this morning after not getting a call from my mail order prescription service. I asked where they sent it. Well they sent it to the old mail-order service.

So I guess the paperwork I filled out at the first appointment of the year for all of us. I guess mine wasn't updated on their computers, so now they're calling it into the local pharmacy. But I must admit they called me about 5 minutes after the message was left, that was impressive.

Don't get me wrong I think they're doing all right. But is there room for improvement absolutely.

Visits with Dr. Crumpacker are always uplifting and enlightening. I come away with a better attitude than before the visit. Life, in general, is better. I look forward to my next appointment with him.

He is a doctor that really cares about you and how to make you better

21st Century Medicine is practiced here. The results can be, and were in my case, simply life-changing. I encourage all those suffering from either mental illness or neurological disorders of any kind not to give up their dream of relief.

Dr Crumpacker is by far the most effective.pleasant and caring psychiatrist we have ever met. My wife has suffered from depression for years and suffered greatly until we met Dr. Crumpacker. With his guidance and care our lives have totally changed for the better. I thank God every day for the help he has provided us.
