Received an out-of-network bill from this doctor for a standard hearing test (preventative care) that was performed on my baby. They did not state that they're out-of-network at the time of service nor did they offer any other options. Also, it was a different person that actually performed the test! My insurance still covered the standard rate for this service but this provider is still chasing after me for "balance billing".
