My grandparents have lived at this community for some time now and love it there! I know they are happy and taken care of at Legacy!

Legacy Oaks of Azle (Owner) please speak to Ceri Johnson, Division VP Operations; Sarah Shaw, Regional Director of Wellness; Chad Hoffman, Regional VP; and/or Aaron Nordell, Regional Director of Operations about our well-documented concerns. There have been several other Executive Directors and Wellness Directors in our experience, but they're no longer at the community.

Original review: Our family experience with Civitas has been very similar to the frustration expressed by Kristine Stapleton. The corporate responded to her review with "we truly appreciate you bringing your concerns to our attention", but I suspect she had already repeatedly voiced her concerns to those same people.

Please read the reviews of people who have actually had their loved ones in this facility - assisted living or memory care - rather than people touring the facility or businesses having networking meetings here. I can't speak for independent living, but assisted living and memory care are not 5 star except for a few exceptional caregivers.

In my experience, first impressions can be very misleading with the right "marketing".

I have been with legacy oaks of azle for about 6 months and this is by far the best place I have worked, the managment cares for their employees and the residents. It’s a blessing to work here. Top of the line company to be apart with and grow with. I always get acknowledged for my work, seriously the best company I’ve worked for.

I have been with the company 4 Years. Nicole our new executive director is wonderful! Lauren our wellness director is absolutely amazing and cares deeply for our residents! I couldn’t ask for better bosses! I’m happy to be apart of the legacy oaks team!

I’ve worked at LOA for almost six months now. I can say, this is the best place I’ve worked at as a caregiver. I love the team work in memory care from 6am to 2pm.
I started on Assistant living and moved to memory care about two to three months ago. I have 7 years experience as a caregiver. This job is my passion along with being a girl mom to a first grader. I’m also fortunate that I can bring my dog to memory care he sure loves loving on everyone. I give my loyalty to LOA because they make me feel welcomed and appreciated. Our administrator Nicole is always helping out each department. Once I saw her working the dishwasher in her nice clothes, that says a lot! She is amazing and always there to help when it’s needed! Same goes for her assistant Laura, she is always there to help out in assistant living and in dinning room. Our dining room is beautiful, the kitchen has high quality foods and serves 3 meals a day. We have fun Activities and events at LOA too. Last but not least, Our wellness director Lauren and assistant Jordyn are so wonderful and care so much for not only the residents but their employees too. I don’t know what I would do without them! I couldn’t be more blessed with the bosses I have.

I've worked at LOA for nearly 15 months and with nearly 9 years experience in this field, after working in multiple different settings and facilities similar to LOA, I can without a doubt say LOA is top notch.
The wellness director (Lauren) and assistant wellness director (Jordyn) have the utmost respect for their team of aides and med techs and their appreciation for their team is well vocalized. Lauren and Jordyn go out of their way to ensure the wellness team has the tools, the knowledge and the confidence to take care of residents with dignity, compassion and respect and I have the utmost respect for them as they ensure these standards are adhered to; not only by modeling it themselves on a daily basis but also by holding staff accountable if staff fails to provide compassionate, dignified care. These two ladies are a strong force in keeping residents happy, healthy and safe.
The facility is clean, well kept and housekeeping staff does a phenomenal job at what they do - providing room service as well as keeping the common areas cleana and welcoming.
The kitchen provides 3 full meals a day to residents, complete with a full menu offering beverages, appetizers, an entree and deserts.
From the reception desk, to the maintenance department, to the aides and med techs you'll see in the halls, as well as Lauren and Jordyn, you'll always be greeted with a happy, smiling face, eager to assist you in caring for your loved one with an emphasis on compassion, respect and dignity.

Kelly at Legacy Oaks of Azle has gone above and beyond in helping me find the right place for my Mom. Her outstanding professionalism, experience, compassion and empathy for my situation was and continues to be comforting. Prior to my Mom living at Legacy Oaks of Azle, Kelly would be only a phone call away for me as our circumstances were evolving. In those uncertain moments, Kelly was very reassuring making this transition of life a calm one.

Moved my dad into Legacy Oaks in February. His initial evaluation was done by someone other than the wellness director. Upon his arrival, my dad was tired as he was moved from his prior assisted living in California to a plane ride and my home for two days. That is a lot for an 80 year-old. The wellness director was not overly happy with a new tired confused resident. I was assured this shiny newer facility was going to take good care of him.
Within a week of being at Legacy Oaks, my dad had a fractured shoulder (dominate arm). From there the experience at Legacy Oaks became worse:
1. After discharge from hospital and initial rehab, the staff was unwilling or unable to assist my dad due to only having one working arm (other was supposed to be stabilized). They became inpatient with my dad on a number of occasions, only adding to his fear and confusion.
2. When he was cleared to use his arm again, I requested he go back to rehab to regain strength as the staff at Legacy Oaks was not walking with him and the walker but just put him in a wheelchair for 6 weeks because it was easier for them. The wellness director suggested Azle Manor.
3. The wellness director and staff gave us someone else's medication to take to Azle Manor. The wellness director had to personally deliver my dad's correct medication which contained sensitive heart medication (that they failed to make sure we had).
4. Azle Manor REQUIRED families to use their pharmacy or pay a $350 a month penalty. So if they screw it up, you have no ability to help your loved one get important medication from a local pharmacy.
5. Due to Azle Manor’s refusal to give sensitive heart medication to my dad for 7 days, I moved my dad back to the initial rehab that took wonderful care of him. The Legacy Oaks wellness director was clearly upset with us that we moved from her preferred location despite Azle Manor placing my dad in a life threatening situation.
6. The Legacy Oaks wellness director did her re-entry exam of my dad and deemed him memory care necessary. Interestingly, the staff at the rehab was the same staff as his initial visit and were raving about how much he improved.
7. Trust in any care that my dad received Legacy Oaks was gone. His experience there was not one of caring concerned staff. More like, let’s just get through the day and do the bare minimum.
Moved my dad from Legacy Oaks facility immediately. If your parent/loved one is not fully able bodied, do not place them at Legacy Oaks. It is not a compassionate, loving facility they like to make you believe.

My mother is in the memory care side. They do a great job of caring for her.
If anybody from corporate ever reads this review, you need to give your "boots-on-the-ground" people a raise.

My mom has lived in the independent living there since May of 2020. Overall she's happy there. Whoever built it didn't put public restrooms on each floor which can be a big inconvenience not only to the residents but visitors.

Visited Legacy Oaks this week. Beautiful facility, friendly and knowledgeable staff, residents seem to be happy and well cared for. Very good to see.

Amanda Dudziak, Community Relations Director, was a great host. A group of business professional were involved in the networking meeting and then we ent on a tour of the campus. This is an awesome independent living facility for seniors. Amanda says they is apartments available.

My parents and I met with Amanda D to tour the facility to see if it would be a good fit for both of my parents. Amanda was very gracious and patient in answering all of our questions. The facility is wonderful. My parents are very excited to move into their new apartment soon and are looking forward to participating in some of the many activities that are planned.

Thank you SO much Amanda D for helping make this transition as smooth as possible for them.

This facility is next-level "great. " We started our experience with my mother-in-law who was ultimately moved to memory care within the campus. During the time she was there my own mother visited - and we toured Independent Living. After the tour, I was able to convince my mother to move in to an Independent Living Apartment.

The move-in process has been smooth and staff has been incredibly helpful. From Amanda and Laura in the business office (who helped us with every little question and concern) to Darla and Ray (who run things behind the scenes), to Jaycee (who's a smiling face willing to help anytime you need it), right down to the ladies at the front desk, (especially Sharon), who are there to help and reassure residents when something is going right - or when something is going wrong.

The staff is incredibly responsive to resident needs. The facility is clean. The apartments are beautiful. The cleaning staff is friendly (and I've met several of them as well - especially Gracie).

The food is tasty and portions are appropriate. The only complaint we have is that ordering food for carryout back to an apartment tends to take a little longer than one might think it should (but there are staffing issues that cause some things to happen out of their control).

Overall, this facility makes me feel very grateful my mom is able to live there. She is safe, happy, and has everything she needs within the campus (including covered parking and coded keypad entry for after hours access).

This facility is priced fairly. Although a little expensive, they take care of every single thing your loved one needs, and that's worth every penny!

Legacy Oaks of Azle is an amazing Senior Assisted Living Community.
We continue to have the pleasure of working with and bringing our senior clients to tour this treasure of a community that has everything a senior needs to be happy and well taken care of in every way:
