San Antonio Wellness and Rehab is an awesome place to have any loved one who needs skilled nursing or long-term nursing care. Javier and his team really care about the resident's wellbeing and daily lives. They go above and beyond to make every stay, as good as can be.

The Nurses and Admen where nice and helpful. Very mindful when changing dressings on wounds. Have to admit they were a little slow responding to the call button at times, could have been short staffed for graveyard shift. Overall I was pleased with the facility and the help I received, they even provided some clothes for me as I came straight from the hospital in a gown and didn't know what I needed.

My mother died under their care after 15 hours. At two separate times they were negligent in their care. The first was when my mother was moved to rehab following a hospital stay. She developed a UTI and after requesting a doctor and antibiotics she was provided neither. They called an ambulance on the third day because she would not wake up. She was unconscious for 3 days and the hospital said it was due to the severity of her infection. The last time was their failure to provide oxygen in a timely manner. It took almost an hour to get her oxygen letting oxygen level to drop to 53%.

On entering, main door was electronically locked.waited for front desk to let me in. Front desk was very nice and helpful in registration process to get admission pass and assisted ne in pointing to my brothers room. It was my first visit. The entire facility was busy with staff assisting other residents and or assisting other staff in coordinating tasks to be accomplished. Patents that were ambulatory were clean and most parted in designated areas to accommodate social interactions. Very clean facility. Brothers room was single occupancy and clean. Impressed overall.

Everyone was so helpful and so caring of all your needs. The two young men that work there were so helpful and assisted me with all of my things into my room. The front desk lady Sylvia she was the sweetest ever and let me know everything I needed to know and where to go. Thanks S. A. Wellness Center.

Update: I just did my interview. Facility is nice, pt feedback not so much, and for the state of TX is a shock to me…nvr in my 18 years of CNA (WA or HI) have I had to be responsible to geriatric care for 20 patients per shift. That ratio 1: 18 or 1: 20 is outrageous and no wonder family members have issues with their loved ones. Find a facility where the ratio is smaller. Even my recertification training nursing home the ratio was not this bad.

As a CNA over the years and what I’ve experienced is that there may be some employees that need retraining for interpersonal skill training to be compassionate towards all patients and correct patient care (lifting/transferring/etc). Patients always have their own rights and can file a complaint. But as a CNA we are all overworked. Sometimes facilities don’t have good communication within all of the staff and that is prob why they got new mngmnt. Or they are short staffed, not payed enough for the many yrs experience/amnt of work they have, are also set in their own way and make short cuts, and they just try to do their best with what’s avail. But by no means should there be any abuse towards a patient. The facility is beautiful and I believe they have showering aides which takes less stress off of the CNAs. There are also hospitality aides that are there to help by passing water and doing little tasks, but cannot and ARE NOT licensed to do patient care. Mngmnt had changed to new ppl earlier this year. Everything for every patient should be noted into charts to pass along to nxt shift. As a new incoming patient sometimes it takes a while for their personal care chart to be made, but most times it should be prepped prior to arrival by the admitting nurse and added to with any changes by the nursing staff. Any dietary info should immediately be passed along to the kitchen staff to relinquish mistakes. Sometimes mistakes are made but we can only learn by them and correct those ways. Their mngmnt has been super nice so far.

This place is the worst. They only give patients a bath twice a week and they never answer the call button until hour's later. I plan to reach out to the news on them. The sign states you can request a bath. I requested one for my Mom and they still did not do it. I spoke with the Administrator who said he would make sure it was done. It still was not done until the day she was schedule to get a bath. Last but not least my Mom's nurse is also very rude. This is ridiculous and I will not have it.

Wonderful staff and my Aunt is happy their and she is doing well with the therapy they know what they are doing. Great staff and they are tending to her ever needs.

My mother has been in this facility more than once. Last time was a couple weeks ago the time before that about a year. They have way too many patients and not enough people to care for them. I was not happy at all. I went there one time it was already almost 2 in the afternoon. They hadn’t even served her lunch. And then when they brought lunch and they just put it on the table and walked out, didn’t even say do you need help nothing and I’m sure with me not being there it would’ve been even worse. There was just several things that I did not like at all that could’ve been changed.

Lots of miscommunication, an hour and a half of staying dirty. Staff dropped me, not fully trained. No diabetic diet

Beautiful & clean facility. The staff is caring and professional. Most importantly, the residents seem happy & cared for.

Everybody was kind and polite,
inviting also. The facility looked clean. They have a good safety measures all around. It seems to be a good place.

Everything and everyone was great the only problem was the guy at the front desk forcing us to wear useless masks that was not cool but the nurses taking care of my father were very polite and I'm super grateful for them

Beware and stay away from this place! Check in my father and within the hour, I knew we made a big mistake!
Dead bugs all over the closet, spider webs everywhere-took them 3 hours to come clean and no apology. Call button malfunctioned. Provided no pain meds until 3 hrs into stay, no hygiene assistance in timely manner. Rude Assistant Director of Nursing was very unwelcoming and we knew it was in retaliation to us voicing our concerns. No special diet for renal patients-he was served a slice of ham loaded with sodium the first night. We couldn’t get out of there fast enough! Nearly 48 hrs of uncompassionate and almost inhumane care. How does one go from a hospital care where the goal is to keep the pain at bay to a place where pain is at the bottom of the totem pole?
No, don’t check your loved one in here—-run the other way!

Don't ever take your family here! My mom is on hospice. She needed respite care. Day nurses were kind, night nurses made her feel like she was a burden and got upset with her. She was extremely upset that a patient "wandered into her room" startled her, went through her belongings, taking some items. I ask where are the nurses? What happened, never got a straight answer. My mom deserves better care! The fact the business office asked for 10k a month was a joke. No one charges 10k. Not to mention when they asked for personal information to see what she may qualify for I was asked to sign a "waiver of right to cancel for funeral benefits contracts". After legal advice was sought, that's just another way to scam an elder person out of money. Remind your loved ones to get a long term policy with long termcare. Medicare and AARP don't cover anything. United Healthcare referred us to Heartland.
