DO NOT CONSIDER THIS FACILITY IF YOUR LOVED ONE NEEDS EXTREME CARE! My dad was sent out here for physical therapy & rehab from the hospital, this place is full of the most careless people. My dad was bed ridden & could not do stuff for himself. My family & i would call the facility and they never wanted to help him get in contact with us (helping him dial his cell phone since he couldnt do for himself) not once did they actually give him physical therapy. MIND YOU HE WAS ONLY HERE FOR WHAT SEEMED TO US LIKE A VERY LONG 7 DAYS, thank god we got him out of this horrible place i feel very bad for the elderly that get thrown out to this careless place the only good person i met was the front desk lady.very nice lady. The other nurses looked at you like you were from outer space and were very rude

West Oaks Nursing - I personally know Adam Carnes and know without a doubt that this facility is an amazing place and is capable of great things.

West Oaks Nursing and Rehabilitation Center is a first class Facility.
The residents are treated with love and respect. The management staff are second to none.
The Nursing staff are tops in every category. Special shout out too the other staff members.

This was my first visit and I was impressed with the facility and staff. The residents were very kind as well. I love the overall atmosphere.

Not trustworthy. The staff is messy especially the administrator Sandra and the Adon Greg. They talk behind patients and family members back. And the administrator does not call in allegations of abuse or neglect, she doesn't report to state if she decides it's not abuse or neglect. So state never get the self reports. Something not right about that. Watch your back if they lie to you they will lie on you.

I am currently a resident here at West Oaks. On November 19,2023, I will celebrate my 2 year anniversary. This following
has been my journey.
When I arrived, I wasn't able to walk or take care of myself at all.
I weighed 70 lbs, and was suffering from a severe leg wound. I was placed on high calorie diet and immediately began daily wound care.
I have doubled my weight And my leg is almost healed. I now walk and my physical, mental and spiritual well being is wonderful!
All of this could never have been accomplished
without the wonderful Medical, Rehabilitation and Administrative staff here at West Oaks. I am so grateful for my experience here.
I do not know what the future holds, but I trust my life with West Oaks Nursing and Rehabilitation!

So, like beautiful97’s review from four months ago my situation was very similar with my mother. We weren’t notified about injuries or issues until after the fact (when they worsened or got bigger) … my mom was bedridden so we weren’t able to see the sores all the time. She had a bedsore that became septic, and she was hospitalized, and that was pretty much the beginning of the end. I am regretful, because the bedsore did not have to get as bad as it did, had we’ve been notified ahead of time. They only started notifying me about the sore once it had been infected and needed intervention. Although the wound doctor and other staff are very helpful… The day-to-day nurses are not helpful at all. They never answer the phone. Don’t fulfill requests such as water or incontinence care. The new Director of nursing Cynthia, comes off cold and has no bedside manner. Although she is professional in her verbiage, there is a condescending tone to everything she says… She came in to the facility the last few weeks that my mom was on hospice, and she just gave me a very “yeah yeah this is what happens, prepare for the worst, please wear your visitor sticker, thanks bye” type vibe.

My mom passed Saturday, and instead of calling me about her belongings, they chose to pack them up themselves. I’m just not happy with the level of care provided and if I could change my decisions I would. I did this alone with little guidance, unfortunately I know better now but it’s too late. Please please please anywhere but here if your loved one is not able to speak or stand up for themselves. They favor the “easier” patients.

Cynthia was actually very genuine in offering her condolences when I went to pick up her items. I don’t know if she saw the review; but I did appreciate her concern.

I’m sure it’s a great facility, but our experience was not up to my standard. It wasn’t my definition of great.

My mom had an issue with meals/feeding and they were accommodating in most ways. However, it was like pulling teeth with some staff, while others seemed clueless. MARGARET was great. Very grateful for her solutions in that moment in time. GREG was also great thru-out whole stay from beginning to end. There was good with the bad, but I feel possibly some neglect somewhere.

Please watch your loved ones. Wish somebody would have told me sooner. The have a pattern of intentionally causing issues for the Gtube patients. Yes, staff is doing it. Then send "difficult/problem" patients to the hospital by timing those hospitalizations when their State inspection is coming up. The difficult patients are kept in the hospital longer and off their census so they don't have to acknowledge those patients documentation or care. Their operation is negligent and poorly ran with bad management. The business office keeps patients money from SSI where they are left with a few bucks. The Therapy department documents but never do the therapy on some of the patients they are billing. All they do is go back and falsify the documentation. It is sad, they rather those for patients to die than have to answer to the STATE about their poor care. The Administrator and Social Worker gets the 'difficult' patients to be put on HOSPICE. Once hospice is in place, those patients decline due to forgetting about them and they die fast. Even the DON, as a licensed minister, is a hypocrite and knows all too well how to paperwork and cheat the State. Yes corporate knows what is going on. If they know you know, they make it difficult for you and turn against you. Go back and ask for records and the names of staff that documented on your loved ones.

This was my first time to visit my loved one; I was impressed with how clean and fresh the facility was; the staff they I came in contact with was very nice and helpful.

I was called for interview RN position in this facility. The interviewer (Kathy) insulted my personality. She said someone told her that nursing profession in my country doesn't worth it. Even if someone told you that, why can't you go ahead and conduct your research. If you care to know, some nurses in my country have diploma, degree, masters and PhD. Some had passed nclex and they are waiting for the Visa to be approved if not for the pandemic issue. Some have been working here and they are doing good. If nursing profession doesn't worth it in my country, why would USA and UK recruit nurses from there. Individual nursing education were evaluated and licensing were verified before att for nclex exam. I know I'm newly licensed and I need the experience that was the reason I applied to your facility. You as an interviewer did you have your experince overnight or built your experience in a day. Even if you are not going to offer me the job, that doesn't give you the audacity to insult and embarrassed me. In one word, your statement is derogatory. When you placed out advert for job openings, try and list the countries that you want your candidates to come from.

I don't need you to meet my expectation neither do I need your service. If Kathy is still in your facility tell her and warn her to always tame her tongue and learn to respect people.

I received a call from Hannah today and she explained what was going on over the weekend. The nurse who responded to me went to my friends room Saturday evening and gave me an update on her. I am so appreciative of their care and professionalism. I got myself worked up for nothing. My deepest apologies
