Forme offers a wide range of services- I have been here for urgent care needs as well as COVID testing. The facility is welcoming, spacious and clean. The team is knowledgable, efficient, and friendly. Would highly recommend!

Where do I begin? Rude staff all around. Young and unprofessional. My mother has been coming here for years and it seems there’s a new charge every time, even with their own membership. Dr. Gary is a whole other story. Always rude and never on time for his appointments. Stay away from this scam of a place!

Looks very professional and nice. Their staff is younger- less experienced looking fyi. I waited an hour just to even be seen by somebody. For urgent care they had quite a bit of staff available and nobody in the waiting room yet they seemed in a rush with me

I came here 3 times in total. One for my green card physical exam. One for COVID testing for my sister. In both times, my experience has been very good except a long waiting time (much longer for the exam and less for the testing). The staff has been usually very polite, kind and courteous.

This is the third time I am coming here. A family member of mine is going to fly internationally in 2 days and we needed COVID testing. We got the appt 2-3 days ago and they informed me on the phone that it was going to cost 25$ out of pocket. We came today and right when we are entering, they say it is 75$. They had a meeting yesterday and changed the pricing with no notification to us. And the staff who was telling me that was not nice, saying it is not his job to let me know. This is very frustrating.

Went for a COVID test, Sunday morning no line, filled out forms, my test was taken at exactly the time of my appointment. By the time I got home I received a call with my test result. Kudos to the staff. If your looking for parking, stay in the right lane, pass Forme and make a right into the City Center parking lot.

Do not get tested for COVID-19 here. They charge $25 and say it will take 3-4 business days to get results which is less time than the free version which you can get at CityMD. I have been waiting for 7 business days already and feel like I threw out the $25. Go to CityMD instead.

I usually don't write reviews but this time I'm very upset and disappointed, my mother was seen on Saturday for an emergency visit due to not feeling well. Although having all the symptoms of the flu, the doctor sent her home with a work note and pain meds. She was not feeling any better sunday morning. We took her to the hospital and to our surprise she was diagnosed with the flu and additional tests are happening at the moment. My mother mentioned that Saturday the office was pretty busy and lots of people were there. It seems as that the doctors didnt give the attention as needed to the patients and did not give quality treatment to any of them. Hopefully no other person has been misdiagnosed.

Billing department doesn’t know what they’re doing. I summited two forms of insurance one primary and one secondary. Was never informed of outstanding balance. They failed to send my co-pay to my secondary insurance company.

Worse medical services I've encountered, went here on Tuesday with an excruciating pain on my foot I was told that there was no podiatrist but another doctor could see me, after waiting 50min in a empty waiting room I was called in by one of the staff, once the doctor came in I tried to explain what was wrong and even expressed that I did some research which was turned down by the doctor by saying "oh well if you are a doctor I don't know why you're here" super unprofessional. After that I was told I needed an x-ray 2 people checked and just said that" the X-ray guy had left" and I would have to come on Friday because thats the only other day they do X-rays after leaving went to pick up my prescription at my local pharmacy and it wasn't anything through the system I waited a day to get my medicine and they still didn't send it after calling the office to figure out what's going on I was put on hold for 10min and after I was asked my information they hang up on me. Avoid this place if you are looking for good medical services!

From the minute I walked in, I felt that I was working with experts. The facilities are beautiful and the care matched the quality they are known for. I was seen by Jennifer who asked all the right questions and made me feel comfortable and cared for.

Not too bright here - tried to tell them someone was spoofing their phone number and representing themselves to be a company giving away Marriott Vacations - got transferred 4 times and still didn't get a competent human - - - if this is any representation of their other services I would think twice

The customer service is ok. I went there about a year ago and saw three different doctors. Their so called "doctors" seemed very incompetent and acted like interns, not real doctors. I'd never go there again.

Formé is awesome! Excellent providers and super friendly staff. The unique part is they have many different specialties including Primary and Urgent Care, Physical Therapy, Gynecology, Podiatry, Orthopedics, Neurology, Behavioral Health, Diagnostic Testing, Aesthetics and Rejuvenation services. Not your average "Urgent Care"!

I know Dr. Midelton for 19 year and he’s a great doctor, also i recommend Forme. The best Medical Center in White Plains. The services and prices are affordable.

Recently signed up for the medical membership, and have visited twice so far. Super impressed with the quality of care from the physicians and assistants to the professionalism and courteous service received at the front desk. Kudos Forme', and thank you for quality medical care that I can afford.

So thankful for this facility. I've been there multiple times in the past year and have always experienced outstanding medical attention. The staff is is wonderful and the facility has state of the art technology. I love this place!

Went to Forme for an inner ear infection last week and can’t say enough good things about this place.
From the front desk staff to the Medical assistants to the provider it was a great experience considering I was really suffering with ear pain.
