Formé Medical Center and Urgent Care, White Plains NY

The receptionist and the team is very good. But some doctors not listen at all your needs. If you ask about it.they can do nothing is doctors rules and knowledge who give to you the service.

I got the membership a few months ago and I feel very happy with my plan, I am using all the services my plan provides me, and they are really amazing. The medical center is beautiful, and everybody is willing to help. I highly recommend it to those who need a medical plan. Prices are unbelievable and they make you feel at home!

Formè Medical Center is a beautiful facility with a wonderful staff. I saw Dr. Midelton for my back pain and he listened attentively to my concern. I would highly recommend this place to anyone who lives in the Westchester area. Doctors and staff are knowledgeable and professional.

The place is elegant and service was quick and easy. I went in for a consultation and was seen by a doctor shortly after checking in. I’ll be going here for my medical needs from here on out. The staff is also very welcoming and professional.

The most convenient hours! I have been to see the dermatologist several times due to their Sunday hours. The staff is friendly and the facility is spotless.

Incredibly helpful and informative care! I literally changed all of my providers over to Formè after my 3rd urgent care visit. It’s clean, professional and friendly.

Today June 6,2018, I had an emergency and went to this facility. They told me no one was available to see me, as everyone was in a meeting. It was around 12: 16pm, they said I would have to wait until 1: 30pm. You have an urgent care facility that does not have anyone available, in the event someone come in with an emergency. I got back in my car and went to White Plains Hospital emergency, where I received excellent care. I gave FORME 1 star, because it does not allow me to give them less.

This place preys on the local immigrant community. They are discriminatory towards what insurance plans they take. Making patients of low income pay $100.00 per visit. To add insult to injury they were unprofessional during my brief encounter with them. Don't go here there are far better urgent medical care offices within walking distance.

This practice and service is terrible. I found this doctor office/urgent care online primarly for a PCP. I called and made an appointment and when I arrived early, I end up waiting for 40 mins. I was not even seen by the doctor and I had to leave due to another appointment I had to be at that evening. Upon rescheduling, again I showed up early for my appointment and this time, I ended up waiting for one hour and still I was not seen.

I spoke with the young man at the front desk and I asked him to clarify how their set appointments are in relevance to walk-in urgent care patients. All he had to say was "I'm sorry but, the doctor is really backed up. "

I do not think that a patient whom is scheduled for an appointment should have to wait over 20 mins, if a patient whom comes in is not of urgency. Forme Urgent Care/Medical Center should not combine the two. A patient whom is coming in for one visit, should not have to wait for urgent care patients be seen. If the matter was that much of an urgent matter, that is what the ER is for.

Let me just make this clear, I used to work for an Urgent Care and I am very familiar with the patients whom come into the office. More than half of the cases are not of an urgent matter.

Highly recommend this facility. The office space is modern, clean and makes you feel like your at a Spa. The front office staff is amiable and attentive. The provider listened to my symptoms; I didn't feel rushed at all, and he provided excellent care, my symptoms started to improve within a day or two. The wait time is similar to any other doctors office, so I can't complain much about that. I plan on returning; the location is convenient for me, and the service was second to none.

Highly recommend! Great staff. Beautiful facility. I was taken care of expeditiously and everyone was extremely friendly. Definitely going back.

Forme Medical is by far the best urgent care facility I have attended. From the moment I walked in, the whole process was a breeze! The front desk receptionists were so kind and informative. I didn't have to wait a long time to be seen, and the doctor was took care of me was terrific! This will defintely be my new go to urgent care for when I need it! Thank you everyone at ForMe for your professionalism and care!

Terrible terrible terrible! Beyond unprofessional never seen anything like it lied over 3 times to me about when my blood work would be I’m uninsured and payed out of pocket they made me pay 2 times bc they said the blood work was not good enough never heard that one before

Clean offices & waiting area. Fast service; treated for asthma related symptoms in a courteous and respectful manner. No extra charges. Rx was sent to pharmacy. No problems whatsoever.
