The Carlyle at Stonebridge Park, Southlake TX

The services that the nursing staff render are absolutely outstanding. The aides are very helpful and prompt when you need them. Sally, in laundry is top notch. Every week she always does my laundry early in the morning and brings it back within a couple of hours. Teresa, the Activity Director always has everything perfectly planned for us. She fills up the activity calendar well. I enjoy her personality and cheerfulness. The front office staff is always very nice and helpful.

My grandmother was placed in this facility, I was impressed with Jaci who answered all of my questions and answered the phone anytime I called. Jonathan handled any concerns I had right away. My grandmother said the staff was nice and accommodating. Beautiful facility and great people, would definitely recommend!

I do NOT recommend this place! They did NOT care for my mother in the way an "Upscale Rehab facility" should! NO COMMUNICATION, NO CARE FOR MY MOM and constant headaches when we wanted them to do something to help my mom! Careless people who are ready to "THROW OUT" your loved one when medicare and Medicaid stops paying! To them, "It's ALL ABOUT THE MONEY" Typical rich people BS! I am going to FILE A COMPLAINT WITH THE STATE OF TEXAS AND CALL MY INSIDE CONNECTION TO FOX 4 NEWS SO THAT THIS TRAVESTY IS EXPOSED!

My mom was transferred here from another facility and I am so happy and blessed that she was! The staff at The Carlyle have been absolutely amazing and on top of everything making it a smooth transition for my moms care. Jaci, Tiffany, Jacob and Brent (he is so awesome in making sure my moms smart phone was working!) are just a few of the hardworking, true caring people. They always email me and call me and make time to make sure I understand all things regarding care, doctors, therapy, insurance etc, This facility is clean and well organized and truly put their patients/guests first. Luise and Salta the nurses there are absolute angels and blessings as they care for my moms needs. - Grateful for a warm, well organized, beautiful place!

I have been a resident here about 9 months now and I love it. Therapy has helped me tremendously in getting stronger. The staff is excellent, I would recommend to any of my friends or family. I am very happy that my niece was able to find The Carlyle. Teresa, the Activity Director and Savannah in Medical Records go above and beyond for me every day, they always make sure I have everything I need.

Expensive and care given is not acceptable. Meds are given incorrectly or missed, can’t get questions answered, not responsive when problems are cited.

The Carlyle's therapists were excellent. They were very caring and patient with my wife. Their staff is very nice and friendly. Their food is also very great. I would bring my wife back if she had to go to another facility in the future.

This facility has done a complete 180. The care team is fantastic. My mother in law has nothing but wonderful things to say about everyone. All of our issue have been handled immediately. I couldn’t imagine a better place for our loved one.

Mother was there twice. Place smells like urine. Food is awful. Under staffed. Social worker was unprofessional. Some of the staff (PT, nurses, Occupational Therapist) were dedicated and high quality but one can only do so much when the Carlyle group sucks every penny into their profit making business. That is really what you have to think about.do you want to put your parent in the care of an entity that has making money it’s primary objective.

This place is horribly understaffed. Very bad care ans service. The nursing incharge only complains about the management and never helps out. No one seems to be incharge. Do not place your loved ones here.

Not sure if this review is going to do any thing as the previous reviews have not improved the facilities for this rehabilitation center.

The 20 days my Mom spent here for rehab has been a positive experience. The food was good and the Nurses do a great job knowing and caring for the patients. The place is clean and smells good too.

If the need arises we would choose The Carlyle at Stonebridge Park again.

Suggested we just let my mom die! We switched her to different facility and she recovered and is doing good!

Deplorable. My mom spent one night there and that was too much. Marilyn, the "Director" is an idiot. No breathing treatments for my mom with pneumonia, unsanitary practices, understaffed, and the list goes on. I suggested to Marilyn that she may be better suited to operate a laundromat. That may even be too involved for her abilities. Don't be fooled by the zip code of this location or the pictures. It is a complete and utter disaster.

The Carlyle is lovely. Food is good and the staff works hard. Unfortunately, they are severely understaffed. I feel for the kind assistants who have too many people to get around to! Overall, our experience has been good. I am hopeful that with a new director, the staffing issue will soon be resolved.

The absolute worst. Let my husband fall & fracture 3 ribs. Sent him home with another lady’s injection meds. Zonked him out. Never shaved him. Maybe bathed him twice in 4 weeks. Staff is from Africa and not understttand Horrible

Horrible place, don't let your loved ones go there. My mother was there for therapy (which most of the time she didn't receive). The worst incident was when I received a call from my mother at 3 AM asking me to come over and change her diaper for her, because an employee was having a meltdown - slamming things and speaking in broken english about her "break" she was going to take no matter what. Luckily, I was only 5 minutes away and walked in to find that that was exactly what was happening. This facility will do the minimum, keep your loved one until Medicare's 100 days run out and then release them. Stay away.

The one positive was that there are some nurses on staff that truly do care, but the minimum wage staff is horrible. Can't really blame them, they are understaffed and underpaid. Management is the real problem.

We just transferred my FIL out of this horrible facility. The staff not only forgot to give out medicine after his back surgery, but when they gave him his pain medication, gave him double the dose he was supposed to have. They brought his meals while he was at physical therapy and let them get cold and never answer the phone or call lights. He waited 40 minutes to be helped to the bathroom and over an hour to move from a chair to his bed. This is thee worst experience we have had in elderly care!

The most horrible place for a person to have to go!
Night nurse Lazy Kris refuses to answer any calls.
He doesn't care how much the patient needs help.
They never have enough aids on staff.
They don't have supplies, such as diapers in stock for the patients.
The Management lies and NEVER returns phone calls.
They don't seem to care about the Horrible Care given to Patients!
DO NOT SEND YOUR LOVED ONES HERE!
STAY AWAY!

Worst place I've had to stay. Took forever for nurses to reply. I have a catheter in I asked over and over to have it emptied no one came it backed up into me so I dumped it on the floor

Wait time for a nurse or tech is out of hand.longest time so far is 55 minutes. Medicare rating.should be looked at with a jaundiced (sp) eye. Our personal experience is a grade FFF
